Re, methods - I note that you will be doing a survey first and add in some qualitative questions - keep these simple for ease of response.  I note that you are also conducting separate in-depth interviews as your main qualitative method, which is fine.  
Regarding your quants survey, I suggest you add 'age' before category and ask them to 'please tick box which is closest to ... In your second question, your ask them to number 1 (most important) - there seems to be 6 categories - so  state ' please tick number from 1-6 in order of importance to you, and perhaps include grid with  numbers 1-6, with each aspect listed ,e,g,
taste
Delivery flexibility
etc
Explain  what you mean by delivery flexibility, and personalisation, etc in a way they can be easily understood by respondents.
Re q 3: rate our service, again I suggest you put in list/  grid format (this will also make it easier for SPSS inputting).  You haven't included 'health benefits' in this q. Do you need to?
Your qs for qual in-depth interviews look fine, and you might think of others when quants piece is done.
Clear to see what you are researching and how you will research it in your primary material. You haven't referred to what secondary research you will be doing - I suggest you look at some literature on customer satisfaction for this.
