         <description><![CDATA[<p>Heart failure is a common medical condition individuals face, meaning the heart isn't pumping as well as it should which entail requires complex medical management and patient centered care (American Heart Association, 2025). Heart failure can be associated with increased mortality and morbidity, readmission rates, and costs, therefore promoting the need of ICT to evaluate outcome data, develop strategies to reduce risks, improve health outcomes, and enhance the accessibility of care. Heart failure can greatly reduce the quality of life for individuals resulting in frequent hospital readmissions, poor disease management, and financial burden. Communication and information technology is strongly beneficial in monitoring disease managment, encfourging patient engagement, reducing hospital readmissions, enhance access of care, and overall improve patient outcomes. </p>]]></description>

         <description><![CDATA[<p>Information Technology has been greatly beneficial in the management of heart failure, promoting positive patient outcomes. Information technology can be defined as using hardware, software, services, and supporting infrastructure to manage and deliver information using voice, data, and video or the use of technologies from computing, electronics, and telecommunications to process and distribute information in digital and other forms (McGongigle &amp; Mastrian, 2025). A specific information technology using in heart failure management is known as CardioMEMS Heart Failure System. Closely monitoring a patients heart function and addressing issues before they occur can help prevent hospital readmissions and remain healthier for longer (Rakita, 2021). CardioMEMS helps providers better monitor patients and help them to avoid any worsening systems with more than 20,000 patients receiving the device alone in 2020 (Rakita, 2021). CardioMEMS allows for at home monitoring where information will be directly transmitted to a cardiologist (Rakita, 2021). This device reduces the risk of hospitalizations since interventions can be conducted earlier with a notification of disease progression or incline in symptoms. CardioMEMS in effective in monitoring for signs that heart failure is getting worse by measuring and recording pulmonary artery pressure (Rakita, 2021). CardioMEMS has been a medical device used to promote better outcomes for patients experiencing a diagnosis of heart failure to improve quality of life and disease management. </p>]]></description>

         <description><![CDATA[<p>Communication technology has been strongly supportive in the management and monitoring of patients suffering with heart failure. Communication technology is defined as the use of technology, such as social media, to process and communicate information. It can include email, texting, instant messaging, social networking, tweeting, blogging, or video conferencing. A specific communication technology used to support heart failure is known as cardiac tele-rehabilitation (CTR). CTR is an alternative to centre based CR in which treatment modules are delivered in a patients home environment, using wearable devices and remote communication between healthcare professionals and patients (Tousignant, 2015).  CTR reduces mortality, major adverse cardiac events, hospitalizations, and improves quality of life for heart failure patients (Tousignant, 2015). Cardiac tele-rehabilitation offers an exercise program, education sessions, and medical professional engagement. It has been shown with physical training combined with CR program, patients with heart failure have an increased exercise tolerance and functional capacity, improvement in functional status, reduced dyspnea, improvement in prognosis, and overall increase in quality of life (Tousignant, 2015). This communication technology has been beneficial in reducing risks, improving health outcomes, and enhancing accessibility to care. </p>]]></description>

         <description><![CDATA[<p>The burden of heart failure has been increasing throughout the years with frequent hospital readmissions, increased healthcare costs, and increased morbidity and mortality. The need for ICT is greatly needed to promote positive patient outcomes, promote successful disease management, and reduce risks related to a heart failure prognosis. Approximately 6.7 million Americans over 20 years of age have heart failure with the prevalence expected to rise in 2030 to 8.5 million Americans (Bozkurt, et al., 2023). With the use of CardioMEMS, more than 20,000 patients received the device in 2020 alone (Rakita, 2021). in 2017, a Real World Economic Impact Study revealed a $13K reduction in costs related to heart failure hospitalization costs per patient year (Abbott, n.d.). Another MEMS-HF European Study discovered a reduction in pulmonary artery pressures with a 62% reduction in heart failure hospitalizations revealing improved quality of life (Abbott, n.d.). This outcome data is revealing the benefits of information and communication technology on the disease of heart failure. Overall, ICT has had a major impact on evaluating outcome data, reduce risks, improve health outcomes, and enhance the accessibility of care. The outcome data for cardiac tele- rehabilitation noted a significant increase in clinical and social wellbeing and quality of life during 1 year of participating in the program (Skov, et al., 2021). </p>]]></description>

         <description><![CDATA[<p>A variety of strategies can be used in heart failure to reduce risks, improve health outcomes, and preserve a healthy lifestyle. Strategy #1 is the use of tele-monitoring in heart failure patients. Tele-monitoring offers a range of benefits ranging from decreased hospitalizations, reduced healthcare costs, increased access to care, comfort from monitoring in own home, and close monitoring of disease management. Remote monitoring couples with a system of care engages, informs, and empowers patients for effective home management of heart failure to control symptoms, avoid hospitalizations, and educate the patients perception of illness (Stevenson, et al., 2023). Patients are able to have direct communication with providers from the comfort of their own home making them more likely to participate in care to manage disease progression. Tele-monitoring can improve health outcomes by patients becoming more engaged in their own care, frequent monitoring to assess for adverse events, and provide a means for provider evaluation due to lack of transportation. Heart failure risks will be reduced because providers will be monitoring patients more closely  whether that's through video consultations, implantable devices, patient portals, and/or sensors. Patients can preserve a healthy lifestyle by incorporating Telehealth into their life making it more accommodating to manage their health condition in the comofrt of their own home. </p>]]></description>

         <description><![CDATA[<p>Another strategy in the management of heart failure to reduce risks, improve health outcomes, and preserve a healthy lifestyle is known as the use of artificial intelligence (AI). Algorithms can be created and identified to monitor/manage heart failure symptoms more successfully. The volume of information from wearable devices requires algorithms and computational power to generate information (Hughes, et al., 2023). Patterns of data can be recognized with the use of AI to reduce risks in heart failure exacerbations. AI will overall improve health outcomes by generating new data and monitoring disease management in patients with heart failure. Article intelligence has been on the rise and there can be great benefits in correlation to the management of heart failure. </p>]]></description>
