As healthcare practitioners, active listening is extremely important (Lester and Rogers, 2012). Building a rapport with the patient will allow the patient to feel more comfortable and give us the information we need. Depending on the patient, this rapport can be more difficult to obtain. So, we might need to adapt our way of talking, positioning ourselves and overall attitude to try to match the patients'. Working in care helped me with that aspect as it allowed me to be in contact with a great range of different personalities I had to adapt to.

Reference:
Lester, D. and Rogers, J.R.

The aspect of working in partnership with the patient I struggle the most with is the presentation of risks. Whether it is the risks of the techniques or the adverse effect of the osteopathic treatment. I always feel like the patient will refuse the techniques or the treatment because I won't be able to formulate how little the risk is for them. I am trying to integrate this aspect more into my treatment session to have a real patient partnership. I also need to do more research about the adverse effects, at what percentage do they happen at, and how to justify my choice of techniques

Explaining to the patient what findings we have after our tests will help the patient understand their pain and how we can treat them (Yu et al., 2016). I always make sure to implicate the patient during my tests and make them feel restrictions. Once I gathered all of my needed information, I like to show patients a picture of what I felt and how my findings are related.
I once had the experience with one of my patients that was seen by another practitioner while I was on holiday. This other practitioner did not explain the finding and just went on with the treatment. When I next saw the patient, the pain levels had gone way up. I did ask the patient why they thought it happened, and they told me that the other practitioner did not explain the findings and reasons for the treatment so the patient felt like the other practitioner had less knowledge than me. This reinforced my idea of explanation to the patient.

References:
Yu, H., Côté, P., Southerst, D., Wong, J.J., Varatharajan, S., Shearer, H.M., Gross, D.P., van der Velde, G.M., Carroll, L.J., Mior, S.A., Ameis, A., Jacobs, C.L. and Taylor-Vaisey, A.L.

Consent is very integrated into the English osteopathic world. As I started my studies in France (where consent is not as needed), I had a hard time adapting to this new model and understanding the need for this consent. After a long reflection on my part and observation of my colleagues, I concluded that consent offers a healthier patient-practitioner relationship and it protects the practitioner. I still think that we don't need to ask multiple times to palpate the same area in one session, especially if it isn't a sensitive area. it can make us seem hesitant and can make the treatment an awkward experience. Most patients also ask me to "get started" and skip the consent part of treatment.

Recording this consent protects us. I recently started recording the given consent on the follow-up treatment sheets. I used to only record it on the initial consultation form, but really, consent needs to be given at every session.

Most of the time, to support patients in caring for themselves, we need to discuss their weight. This is always a very difficult part for me as I don't want the patient to feel bad about their weight and I want them to embrace their body shape. Unfortunately, being overweight can cause pain that is increased by weight (Leboeuf-Yde, 2000). I don't want to be seen as rude on non-inclusive, so when a patient talks about their weight and how they would like to lose weight, I always brush the discussion off. Because I am aware that this is not the appropriate attitude to have as a health care practitioner, I plan to go through this discussion with my tutors to have pieces of advice on how to formulate my thought without sounding rude. To help me in this direction, I increased my knowledge of diet through books, But I still don't feel confident enough.
This reflection also goes with mental health for me. I have a hard time indicating to the patient the beneficial effect psychological appointments could have on them and their complaints (Linton and Bergbom, 2011).

I feel confident in referring patients when they need to for every other subject, even sinister ones.

References:
Leboeuf-Yde, C.

As I started my studies in France, I have learn the French treatment model. Treatment is way more global, and so patients usually need to undress up to their underwear to complete full body testing. In the UK, modesty is a very big part of the treatment and we are taught to let the patient decide what they want to take off and what they want to keep. I still struggle asking the patient to take off only a part of their clothing, especially when they are willing to get into their underwear. I do understand the aspect of modesty and I am getting better at it. On some occasions like low back pain, I still think that having the patient in underwear will allow me better palpatory sensations, and so a better and more precise diagnosis and axis of treatment.

It requires us a sufficient knowledge about physiology to know when actions need to be taken.
As I am still a student, I have this beautiful view of osteopathy and how it can help anyone as long as the patient doesn't need a referral. This causes me to try during multiple sessions to improve my patient's complaints and sometimes not have the expected results. I need more research on what the most effective treatment option is for different diagnoses so I can redirect my patient to the best practitioner for them.

When a red flag is present in a patient case, I will immediately refer my patient for another treatment or further examinations as I need to keep my patients safe. Sometimes, I can also get anxious about the treatment of a patient with yellow flags and what further examination. This allowed me to learn a process to be safe and decide whether the patient is a candidate for osteopathy or not.

The best way to improve our patient's care is through feedback.
As I am a student, I ask my observers to write feedback down to enhance my practice. Through feedback, I will be able to have a direct analysis of the attitude toward a patient and how to get better at it. Once I get this feedback, I like to reflect on the negative parts to find a way to modify them and optimally be a better practitioner. Also, I usually compare my feedback to see if a recurring issue is surfacing.
If I have trouble rectifying a negative aspect, I always go to the person who gave me this feedback and ask for advice on how to improve.
I also always acknowledge the positive aspects to keep them positive for my next feedback.
Once I graduate, I would like to leave the option to give feedback anonymously to the patients. That way, this process of getting better can continue even when I am not in school anymore.
Asking for the patients' feedback is also very important. It gives us a direct point of view on how patients would feel during the session. As it might be awkward for the patient to criticize us directly, I think that it would be very interesting to leave the option of anonymous feedback, online or in reception on a piece of paper for example.

As I studied in France, I was taught for three years how to conduct a case history and testing to be as safe as possible with our patients.
For me, missing a red flag was a real worry. Thankfully, after two years of clinic, I feel way better about my knowledge and my ability to spot a red flag. Our tutors are also very inclined to follow our decisions and judgments, which gave me a lot of confidence as it shows trust from my tutors.
I also took on the advice from one of my teachers which helped me be reassured about a case. This advice was to go through the case history again with the patient once I'm done with the questioning and ask if I missed anything. It helps me clear my mind, think about diagnosis and about the information I would need on top of that.
Another piece of advice I was given is to implicate the patient in the diagnosis and treatment by saying "based on the information you have given me". This will give the patient to opportunity to give more information if they forgot to mention something.

Keeping patients from harm include their presenting complaints as well as every piece of equipment present on site.
As mentioned in the OPS B1, I made flashcards, drawings and diagrams to help me memorise red flags, and the anatomy and physiology of the human body. This helps me decide whether the patient is a candidate for osteopathy or the patient needs a referral.
As for the safety on site, it is the responsibility of the clinic and me (as I am a student) to make sure the electric socket are safe and working, as well as the treatment couches, the stools, windows, extinguishers, lights, etc.. and that no sharp objects are brought into the treatment room (PAT Testing, 2017). Even if it is the role of clinic to organise checks of the equipment and change the broken ones, it is the responsibility of the students to inform clinic of any damage made to the clinic.
Keeping the patients from harm also include not going to clinic if we are sick to not risk the transmission of the disease to the patient.
As I am graduating in a couple of months, I must start learning about the safety rule inside a practice to make sure I can apply them correctly once I have my practice.

References:
PAT Testing.

This OPS was widely used during the recent pandemic. Because the hygenic rules changed before I arrived at clinic, it was very easy to adapt to those new rules.
Now that we are out of the pandemic in the sense of the health and safety legislation has come way down, I had a reflection about the general hygiene of clinics and how I feel clean. This is a very personal reflection as everyone has different cleaning standards.
I feel like cleaning the cough with antibacterial sprays improves the general comfort of the patient, it also gets rid of the previous patient's sweat and bacteria. I also think that washing our hands before touching the patient reassures the patient of our hygiene. opening the windows in between patients is also very important. It gets rid of the previous patient's bodily odours and makes the next patient more at ease.
Even if the general cost of the clinic will increase because of the anti-bacterial spray and the window openings, I feel like this will give the patient and myself a better quality of the session and prevent the risk of spreading any diseases.

After multiple sessions without any improvement with one of my colleagues, I had to cover this patient. Seeing how the patient's symptoms were getting worse and after asking additional questions, I decided to send the patient to his GP to get further examinations. This decision was supported by the tutor that comforted me in my thoughts and helped me write my letter.
After the session, I felt confident about my letter and my reasons to refer the patient. Unfortunately, I haven't heard from the patient since.
Interestingly, I was in the room when the patient first came to the clinic and while the practitioner was presenting, I thought about the sinister diagnosis needing further examination. I explained my thought to the tutor who brushed off my suggestion. I was with the same tutor when the letter was written.
I was very proud of myself as my initial diagnosis was the one we ended up taking. Unfortunately for the patient, this diagnosis came on later than it should have because of a lack of details on certain points.

In the particular case of the presenting picture, the patient came after a month of break complaining of neurological symptoms in the hands and feet. After I went to present the case to my tutor, I had to explain to my patient why I didn't want to treat her and why she needed some additional exams for the treatment to be safe.
When I have those discussions with the patient, I usually found myself repeating the same sentence and not giving the patient real useful information. Patients are always very understanding and grateful for the explanation and non-treatment.
I would like to train with tutors to have a better and easier conversation

During a treatment session, it is always nice to have discussions with patients. It can make them feel more at ease and relaxed and it can give us a deeper insight into their life. Patients usually tend to ask questions about your life during this time.
I once had an experience with a patient that was asking quite a lot of questions about me, where I lived, who I was leaving with, if I had a lot of friends or if I was alone in the UK, etc. As those questions were spread out over multiple sessions, I didn't realise I was giving too much information to the patient. The tutor asked me how I felt about treating this patient, and then told me he thought the patient was coming to treatments for me rather than the actual treatment.
After reflection on the observation of the patient during the next treatment, I came to the same conclusion. As the patient's complaints were not present anymore, I discharged the patient anyway.
This experience taught me to keep information about myself private, and so, to redirect the questions to the patient instead of answering, or trying to be as vague as possible.

This pain was very known to him as he had it for more than 20 years. Upon further questioning, I found out that the patient had multiple lymphoid nodules in both lungs. Because the patient only found out about those nodules about 2 months prior, he didn't have results from MRI and CT scans yet. I explained that I couldn't treat him because of the nodules and studies showing the negative effects of osteopathic treatment on the development of cancer. The patient understood the reasons and was thankful I was honest with him and did not treat him.
I was happy and confident about my decision and was supported by my tutor.

For that, we are required to hide the patient's name during exam presentations.
