         <title>At my IPPE last year, I was observing someone in the IV hood who stuck themselves with a needle. They immediately were more worried about their job and the extra work they had to do rather than reporting the incident and learning from it. Creating an environment where techs and pharmacists aren&#39;t afraid to report their mistakes is pretty important.</title>
