I say Hamlet due to his hurtful words that he has said to Ophelia earlier in the book, saying how her doesn’t love her, never sent her letters and so on, and for killing Polonius in cold blood (which Ophelia may or might not know). Then with Polonius, that being his death and his words against Ophelia as well earlier in the book as well, with Polonius seemingly “controlling” Ophelia’s decisions and trying to be really strict with her, which Ophelia might not have appreciated at the time, and now might be coming back to her, in a bad way. Although he did not know it was Ophelia's father he killed, he still killed her Father. Part of why it's Hamlet's fault is because throughout the play he has been seemingly toying with her feelings for him. He says he loves her but then says he doesn't. And it is saif by Ophelia that when she lost her virginity to Hamlet she sings "she said "before you got me into bed you promised to marry me" and he says "I would have married you I swear, if you hadn't gone to bed with me. " This seems to suggest he promised her marriage and then went back on his promise. Hamlet was also the person her father told her not to be around and now her father is dead.Even though the blame goes to mostly Hamlet it also goes to Claudius,Polonius himself and Gertrude for having Polonius spy on conversations,something which Claudius talks about saying that had he been behind that curtain he would have died. In both sources,  Opheila is singing songs to herself. She sings “and won't he come again? No no he's dead.” Her brother, Laertes is appalled by her behavior and says “do you see this? Oh god.” Everyone blames Polonius' death for Ophelia's behavior but I don't think that's the only thing causing her to act out. Throughout the play there is a flirtatious tension between Hamlet and Ophelia, in a previous scene Ophelia tells Polonius about her interactions with Hamlet and Polonius warns her to stay away from him. In Act 4 scene 5, Ophelia sings “he let in that girl, and when she left she wasn't a virgin anymore.” I believe she is talking about herself, it is unclear whether Hamlet raped Ophelia or used her for sex, taking her virginity. Claudius' machinations led to Polonius being in the same room as Hamlet at the time of his untimely murder, a murder which was directed for him. This traumatic event is believed to have caused Ophelia's sudden madness. By sending Polonius rather than spying on Hamlet himself, he placed him right in Hamlet's crosshairs, and allowed him to be killed in his place. You could argue that Hamlet is most responsible for Ophelia's madness, as he not only lashed out at her, but also caused the death of her father, Polonius. Nevertheless, the deceitful king ultimately must bear the blame, for Hamlet's madness was spurred by the death of his own father. Who killed Hamlet Sr, you may ask? None other than Claudius. By doing so, he created a domino effect that led to the events of the present, where not only is his right hand man dead, but Ophelia too is left empty and mad.
