I think this is because they need to be able to abstract a little or have higher level thinking skills. This is often a problem with learners who haven't had a higher education. There's a lot of work that needs to go in to get them to this stage.

Reading instructions correctly can be a stumbling block, especially if learners mark an answer with a tick rather than an x etc or forget to turn the page over and miss a page of the exam
Perhaps a workshop on exam technique or managing exam anxiety.
