         <description><![CDATA[<div>10 months old<br>Premature birth<br>Mum refused custody<br>Dropped by foster carer<br>Brought to hospital in an unresponsive state, had a brain surgery, but doctors failed to save her and was put on life support<br>Birth mother wasn't allowed to see Skyla because of her personal history around April 2017 It is unclear if the foster mother in the latest case will be arrested or face any charges. <br><br>Baby P was born on March 1 2006 <strong>&nbsp;</strong>November 2006 <strong>:</strong>&nbsp; Baby P had bruises on his face and chest which were spotted by the GP and &nbsp; Connelly is arrested.<br>5 weeks later the baby was placed back into the care of the mother.<br>Feb 2007: A former social worker Nevres Kemal, sends a letter about her concerns over alleged failings in child protection a month later the CSCI met to discuss the concerns in the letter.<br>April 2006<strong>:</strong> Baby Peter is admitted to North Middlesex hospital with bruises, two black eyes and swelling on the left side of his head.<br>May 2007<strong>:</strong> After seeing marks on the boy's face, a social worker sends Baby Peter to the North Middlesex where 12 areas of bruises and scratches are found. Connelly is re-arrested.<br>Jason owen who was connelly's brother who moved in with a 15 year old girl and was found to be a rapist as he raped a 2 year old child.<br>30 July 2007: Injuries to Baby Peter's face and hands are missed by a social worker&nbsp; after the family deliberately smeared with chocolate to hide them.</div><div>1 August 2007: Baby P is examined at a child development clinic.<br>3 August 2007<strong>:</strong> Baby Peter is found dead in his cot.&nbsp;</div>]]></description>

         <description><![CDATA[<div>Amy Howson was 16 months old.&nbsp;<br>Physically abused and then murdered by her 'violent father'.&nbsp; Her father held her over his knee and broke her back in two. The injury led to spinal shock and rapid unconsciousness and Amy died in hospital hours later. Died because of a broken spine on December 2007. There was a lot of evidence where it stated that her father had a violent behaviour but there was not any evidence which shows that he could have 'a potential to murder her'. She was from Doncaster who has been a focus of concerns about child safeguarding. Seven children known to Doncaster's social services have died as a result of abuse or neglect since 2004.<br><br>&nbsp;She was experiencing physical abuse as&nbsp; she had been 'punched and slapped on numerous occasions in the four weeks before her death, leaving her with fractures to her arms, legs and ribs'. She was also experiencing from domestic abuse because it was happening in the house. Amy was also malnourished and dehydrated at the time of her death, which means that she was also facing neglect.<br><br><br>Police were called to a domestic violence incident involving Howson, but safeguarding officials subsequently failed to carry out a proper assessment.&nbsp;<br>Four months later, Amy's older brother told social workers at his school that he was worried about Howson's violence, but again, officials did not respond.<br>By Howson's behaviour the agencies should have investigated more as the violent behaviour could mean that the children is at risk.<br>Howson, 25, from Nelson Road, Doncaster, was later sentenced to a minimum of 22 years in prison. Amy's mother Tina Hunt, 26, was given a 12-month sentence suspended for two years after admitting to allowing the death of a child.<br><br></div><div><br></div>]]></description>

         <description><![CDATA[<div>Birth name: Shayla O'Brien<br>Born: November 2014<br>Died: 29th May 2016<br>Background: Social services removed her from her birth mother's care at five days old as they believed her mother to be living a chaotic lifestyle and was not in a position to care for her baby.<br>Injuries: 'Baby had injuries likened to those of a 'car crash' victim'.CT scan showed she had bleed on the brain and a postmortem examination showed she had also suffered broken ribs, a fractured  left femur and a fractured skull. 'Dr Harrison said the break to Elsie's tibia would have involved a "twisting mechanism" where her foot would have to have been held, while the break to her femur would have required her knee to have been held'. Her injuries were very typical of those of a shaken baby.<br>Types of abuse: Physical abuse and Domestic abuse<br>Missed opportunities: She had been taken into hospital eight months previous to her death.  Judge Nicola Davis said that their had been a 'tragically missed opportunity' when there had been a failure to identify an earlier injury by medical staff in November 2015, which means that child protection measures were not triggered.</div>]]></description>

         <description><![CDATA[<div>Kathryn smith was a regular cannabis user , she kept her weed in the babies beaker and murdered her 21 month old baby by stamping on her chest, injuries were compared to those being involved in a severe car crash, she was also bleeding on her brain and forensic tests showed that she had a bruised spine. She was sentenced for 24 years that later got reduced to 19.<br><br>She was taken into care for a short period of time in 2013 after she had several unexplained broken ribs and marks on her body, the investigation led nowhere so she was allowed to return home.Social services were involved and health visitors visited the home regularly. after the death of AJ her dad said he felt let down by social services as he called them several weeks before she died to say she was at risk.</div>]]></description>

         <description><![CDATA[<div>HE was four years old and was starved to death by his own mother&nbsp;<br>Died: December 2009<br>Body found: 21st September 2011<br>Hamzah Khans decomposed body was found in his cot for nearly 2 years.&nbsp;<br>The Jury heard Hamzah had a grossly inadequate diet, he suffered from osteoporosis and he was wearing a baby grow of 6-9 month old at the time of his death. Mother Amanda Hutton was a alcoholic and she claimed she had struggled to get her son to eat. Amanda ho was a mother eight other children but neglected 6 of them.  </div>]]></description>

         <description><![CDATA[<div>Keegan Downer was born March 2014, and her mother was addicted to herion even while she was pregnant. When Keegan was born, she was but into ICU for five days to recover. She was then put into foster care with an experienced carer called Jane Murray.&nbsp;<br><br>She was in the foster care home for 10 months and then in January 2015, she was adopted by Kandyce Downer, who was a distant relative of Keegans birth mother. She had 4 other kids which she blamed one of them for the abuse on Keegan Downer. Ms Murray has said that she was surprised when Downer said that Keegan would sleep at night because whilst she was stayed in her care Keegan would wake up at some points.<br><br>Keegan had a lot contacts with professionals. For example a health visitor would come and visit Keegan. The health visitor has reported that Keegan seemed well and showed no signs of distress. Even the nursery had said that Keegan seems fine and happy.<br><br>She was 10 months old when she died, and was battered to death. She died from septicaemia, blunt chest trauma and head injuries.&nbsp;<br>Forensic scientists discovered more than 80 blood spatters on the walls by her cot, more than 30 within the cot, and more on the doors of an under stairs cupboard. She was not only horrifically hurt, but desperately unwell. Untreated breaks led to some of her bones becoming infected. She had a large amount of pus in her pleural cavity. She would have been short of breath, feverish, and suffer headaches.<br><br>This is a clear case of physical abuse to Keegan Downer as well as neglect too. It is also a case of psychological abuse as she tried to blame all of the abuse on one of the other children to try and save herself. The authorities could have stepped in as Kandace had missed a lot of appointments which meant that the hospitals should of seen that something was wrong.</div>]]></description>

         <description><![CDATA[<div><br></div><div>Overview: </div><div><br></div><div>Amy who was seven years old and Owen who was three years old at the time were murdered by their father Brian Philcox on the 17th of June by gassing himself and his children in his Land Rover. Their father killed him so he could “hurt” and “get back” at his wife as she had left him and was in the process of divorcing him and moving out with the children to her mother’s house. She had let him have the kids for the weekend for father’s day. </div><div><br></div><div>Missed Opportunities:</div><ol><li>Friends and family members who were close to Brian Philcox could have noticed changes in his behaviour and noticed him being more stressed and depressed.</li><li>When the mother received those suspicious text messages from Brian she should have called him and if he did not answer she should have alerted authorities such as police to go round to the house.</li></ol><div><br></div><div>Types of abuse: Both Owen and Amy Philcox were not abused however they were murdered.</div><div><br></div>]]></description>
