         <description><![CDATA[<div>Safety requires all members to know the expectations and have the ability to fulfill those tasks.<br><br>Safety requires management support to ensure procedure is followed and it requires team players working towards common goals.<br>That means structure: rules = rewards.<br>They may be intrinsic--self-satisfaction or extrinsic--monetary or public praise.<br><br>Discipline should be fair and corrective to prevent further erring. Blame should be avoided, but rather open disclosure to prevent future incidences.<br><br>Leadership that combines these factors will have a strong committed workforce that thrives on calculated risk, innovation, wellness promotion for patients and self, and stringent checks to ensure no one person uses selfish motivations, or careless activities to place patients, team members or the organisation in harms way.<br>Vivienne Speirs</div>]]></description>

Willingness to commit resources brings safety into reality and is not just lip service.</title>

         <description><![CDATA[<div>I like the concept that cultural transmission occurs, that can then cause the individual to carry on the same safety 'repertoire' in other settings. This is a great description of how humans naturally learn behaviours to either 'fit in ' or survive in a given situation.&nbsp;<br><br>Given this, it is important to have really clear processes in place that outline what is expected, how to manage problems and dangers, how to managed actual harm., how to record and report information. If this is not all in place people naturally default to their own internal ideas, which can cause a minefield of misunderstandings and uncertainty - which is the nemesis of a culture of safety.</div>]]></description>
