This document seems to be created by teachers or HOD’s. This will be based on the curriculum or framework that will be used to educate the students. 

It’s important to have an s&c in order to have a guideline of what has to be taught. Otherwise, failure is inevitable.

It includes the general units of study; the key concepts and associated outcomes; the materials needed; and approximate timeframes. A scope and sequence is also a "fluid" document, which means it constantly changes upon teacher reflection (on student and school needs, etc.).
