For my graphic organizer, I would collect data using surveys the team members would fill out focusing on topics such as rating your team's effectiveness in following the NORMS you put into place, rate the work done so far by your team, rate the success of each team member's role. I would then have some open-ended questions for team members to leave comments regarding what they need from me as an administrator, what items they think would make their team more successful, what tools have made their team successful, etc. The problem solving process is placed after the team has some time to work together and come back together and reflect on their, roles in the team, collaboration, and etc.
