Seeing this patient has a new they had been a previous patient at the clinic before and had received a variety of different treatments in which they found a cranial approach to be most suitable for them. They had received structural approaches before but never got the results that they wanted. The patient informed me about their preferences to the type of treatment they received and as a practitioner I ensured that I listened to their wishes and took on their preferences. I have never treated a patient using a more Functional approach so for this particular patient I decided to ask my tutor to come in and help me with treatment. Upon follow-up appointment with this patient she had received a significant relief from the pain and symptoms as she was experiencing.

I was presented with a 13-year-old patient who came into the clinic with his mother presenting with lower back pain, the patient also discussed to me how he has been struggling with his mental health especially with stress from school as well as having a low self-esteem. In the initial two appointments I finally spoke to his mother as the patient was very quiet and tend to just sit there and listen to what his mother said until one day decided to ask Mother to take a step back and allow me to just talk to the patient one and one. Doing this the patient expressed to me that he wasn’t too concerned about his low back pain and instead needed to talk to someone regarding his mental health and was wondering if there’s anything we could do at the clinic to help him deal with the stress. From this point on I spoke to the patient and gave him the opportunity to talk about what treatment he wanted for the day as we would combined either cranial based techniques to help him with his stress which he found very relieving or if he wanted a more structural based treatment to help him with his low back pain, As well as this sometimes the patient enjoy just talking to somebody and letting out any of the stress he may been feeling from school. The patient shows a significant improvement from his treatments once he was able to take over and control the form of treatment he wanted.

In my first year of being at clinic I had a middle-aged woman as a patient presenting with a lower back pain I briefly spoke her through the treatment that she would receive and continued with the treatment without insuring that she understood the benefits and risks of the treatment. I returned into the clinic the following week in which I was handed the patient’s notes from the reception with a note explaining that the patient had called up two days post treatment experiencing more discomfort than she originally came in for. The next time I saw the patient we did not actually get into any treatment as it was more of a discussion explaining what happened in the treatment And the patient then told me that this will likely be her last session with me as she did not feel like she got the information that she needed. After this session I then had a discussion with my tutor where we went through the process of what happened with this patient and her experience and how it’s needed to be changed in the future and made me realise the importance of discussing adverse effects and risks of treatment and that instant relief is not always provided and coming back take a few days to resolve.

As student practitioners we tend to have the variety of observers when seeing patients To help boost and improve student learning. I have seen a range of patients in which are comfortable with observers and a range a patients who did not feel as comfortable with observers .I recently had a patient a middle-aged lady coming to clinic presenting with shoulder pain are we then sat down and discussed the process of examination at the clinic which involves getting on dressed so that we are able to see the symptomatic area and assess any structures which may also have an influence on this problem. Before the patient came in I made her aware of the observers in the brain in which she was happy to have them but at the same time did not want to take her trousers off only had T-shirt as she felt that was all that was necessary, I respected my patients wish and allowed her to keep the items of clothing on that she wanted and gave her the opportunity to let me know if at any point she wanted the observers to leave. Throughout the examination I sure that the patient felt comfortable and also offered her to have her T-shirt cover her chest area when lying on her side to feel more comfortable and less exposed. At the end of the treatment the patient enclosed that she felt very comfortable in the room and did not feel like she was overexposed At any point.

At this point I knew that there was only so much I could do to help this patient mentally so I decided to have a chat with my tutor to see what they believed would be the most efficient outcome for this patient. After Mine and my tutors discussion we then went back to the patient and his mother where we spoke through a variety of different numbers and websites that the patient could use to get in touch with to discuss his mental health and get the appropriate Treatment and advice needed that as an osteopath I was not qualified to do. At the end of the session I made a list of these websites and numbers for the patient to take home to give him a opportunity to read through the website and see what he believed would be most effective for him. I realise quite late into the treatment of my capabilities with this patient and it’s something that I will be more aware of in the future and will hopefully know when further action is needed again.

In this discussion i give my opinion on prognosis and how many treatments the patient needs based on previous experience and research. I began following this procedure after haring patients tell me stories about practitioners they have seen in the past who made them spend more money than they needed by having too many treatments. Another reason I follow this is because of the previous cases that have reported through GOSC.

Ensuring having a professional patient practitioner relationship is something that at the beginning of being a practitioner I struggled with as I have worked in pubs and bars for the past seven years so the relationship gained was a lot different what I’m typically used to. One of my patients who I saw on a regular basis enclosed a lot of personal information to me as she felt like she needed somebody to talk to but as time went on she started becoming very open to a point where I was being invited to her house for tea. I had a conversation with my tutor about this patient where we discussed that it was beginning to push outside professional boundaries so from this point on I ensured that I reminded my patient of my role as a practitioner and ensured that any information I was giving back to her it was nothing personal where she may have got the wrong impression.

After lockdown in clinic we noticed a higher rate of referrals being carried out due to an increased demand of GP services. Because of this patients were coming to see us osteopaths with any health issues they found concerning. My time in the Eso I have seen a variety of different patients with different medical conditions some of what I needed referrals for further investigations. One Patient I saw came into the clinic presenting with low back pain which we was unable to reproduce symptoms which had insidious onset as well as the patient having a history of Ovarian cancer, Due to the patient’s significant history I thought it was essential to have this patient referred on for further investigations in which I had to write a letter to her GP explaining my concern as well as reporting the patients symptoms and history as affective as possible to get the patient the best care they could possibly receive.

On clinic induction we were provided with a step-by-step tutorial from the head of clinic who educated us on the government rules and regulations when it came to cleaning treatment rooms. This training included how to correctly dispose of PPE, how to effectively take off PPE to prevent further cross contamination, and finally how to clean the room in a way that the virus can no longer be further spread which involved airing the room for 10 minutes between patients.

I had one patient in clinic who came in presenting with a low back pain which we helped ease and manage the pain through manual treatment but the pain continuously returned, this patient was and endomorph meaning he was slightly obese which was something we had to take into account with his management plan. I one-day decided to sit down with the gentleman and have a conversation with him regarding his weight as well as providing him with information on how that may be affecting his lower back pain And advised him to get in touch with another specialist who can help him manage his weight and diet in a more effective way as I’m not qualified to do so. After not seeing the patient for a couple of months he was provided with an exercise plan from a personal trainer as well as dietary changes which he noticed a significant difference in his lower back pain as well as other aches and pains that he never realised were an issue.
