Linguistically, the mother's speech has her own system and do obey some rules. So her English can be a kind of variation of standard English.

The daughter most likely sits on a acrolect region of the cline, whereas the mother sits somewhere in between mesolect and basilect. 
2. There are a lot of un-conjugated or incorrectly conjugated verbs (incorrect tense) that impede on the meaning of what she is saying. There are problems with the sentence structure (sentences can be too short) Sometimes these problems can effect the intelligibility of what she is saying, other times you can get a general idea of what she is trying to say. Chinese cultural and historical references make the text hard to understand from a foreign perspective. 
3. People's varying attitudes reflect the mother's ability to change her position within the cline - people might understand her better when she is talking about a familiar topic, but might have more trouble understanding her, or her grammatical problems are more obvious in the medical example the daughter provided.
