Hi, within LO1 I have discussed the definitions of TBI and how these vary between Acquired Brain Injury, TBI and head injury. I was going to discuss how the variance between definitions could impact the statistical data available - would this be ok to discuss here? Would this cover the critical discussion part or would it be stronger to instead focus on critically discussing data? hope this makes sense?! Thank you!

I am beginning to write about epidemiology and I am collating evidence together, I am finding a lot of evidence around the risk factor of red meats and processed food consumption for increasing risk of CRC, although this doesn't quite link to the case study, would this still be ok to discuss or should this link to the case study and use the family history, smoking and alcohol consumption risk factors?

Hi, I have chosen alcohol as a risk factor for CRC due to Sarah's high drinking habits, most websites advise that alcohol is a risk factor for CRC, although most studies I have found show alcohol has a significant impact on men, but low risk on women, is this correct or am I reading this incorrectly/misunderstanding.

I am focusing on chesney with the clinical investigation being brain stem death, I then wanted to talk about the recognition of deterioration(raised ICP) and the management of this for LO3 (can LO2 and LO3 be switched around to make it flow better) then follow on to IPW with the organ donation team. Am I on the right track with those and meeting all LO's?

Hi - is it ok to use et intubation in recommendation, and then tracheostomy for ongoing management? or is this too similar? would i have to state 'early tracheostomy'

I am working with Sarah case study and looking at nausea and vomiting due to chemotheraphy for the assessment part. Do I need to talk about the a tool such as the MASCC Antiemesis Tool (MAT)? please or the fact that chemotherapy is causing the nausea and vomiting?

Hi, I'm a bit confused about LO2 and LO3. I'm analysing Chesney's ABG and suggesting ventilation as the recommendation of care. Could I then do agitation as the ongoing management part (as this could be related to hypoxia) and discuss pharmacological and nonpharmacological treatment of this? And could I then consider the role of SN-OD for the interdisciplinary working part?

I am discussing the low Hb result for Sarah. I have found evidence behind low Hb and this being caused by tumour ulceration and bleeding, causing chronic blood loss , therefore depleting iron stores and how this attributes to absolute iron deficiency anaemia ... is this ok?

Is it ok to have the recommendation of care as an iron transfusion rather than blood transfusion for a low Hb if the evidence is supportive of this? I know in the case study they do a blood transfusion but I am struggling to find supportive literature for blood transfusion?

Hi, for my clinical investigation i am investigating sarahs low HB levels. I am just starting on this section and looking at evidence, do we assume that Sarah's anaemia is from blood loss due to her CRC or do we have to look at all the other potentials of low hb?

I'm confused how to approach this LO. I am doing it as a standalone section. It is right to discuss the role of the nurse in Sarah's EOL journey and discuss how interprofessional working with cancer CNS, Palliative Care Nurses, Hospice nurses, consultants etc. if done effectively, could optimise outcomes and improve experience?

Or is it more to discuss the different roles involved in Sarah's care throughout her journey, from A & E through to hospice?

Hi, from using the work provided on Moodle, a lot of the work tells us to look at the level of evidence/quality of evidence. Do we need to just be mindful of the evidence of this or do we physically need to write this in our assignments, for example, " this is gold standard evidence" ect i think I am finding this part quite hard.

For Sarah's case study, I have chosen HB as the investigation and I will recommend 1 unit of RBC, I have found research to show that patients who undergo surgery for CRC have a worse outcome if they are anaemic preoperatively, such as higher mortality rates longer stays in hospitals, but nice guidelines recommends 70 - 90 g/l as a recommendation for threshold, would i go against nice guidelines as lots of literature suggests anaemia before surgery is more harmful?

in the epidemiology section for risk factors for Sarah i am doing alcohol, one of the articles provided spoke about the metabolism of alcohol causing carcinogenesis effects, I tried to elaborate and find more research about this, although this is proven hard, the only evidence I can find is studies that were done on rats. Is it ok to mention this but explain that there has been no known research on humans and it is only suggested to have a similar effect on humans? I know it was mentioned not to use anything to do with animals, but I didn't know if i could mention this. I would then go into detail about the studies that find correlation between alcohol and CRC and heavy drinking vs light drinking ect.
