         <title>When arriving the scene, patient was crying and holding his left arm. Patient reports not being able to move the arm, and explains a great deal of pain. The arm is swollen, and I notice a small deformity. There are small abrasions on hand and knee. Patient was not wearing a helmet and does not recall hitting his head. Patient has large bump on head. </title>

         <description><![CDATA[<p><strong>Primary assessment/resuscitation</strong></p><p><br></p><p>General appearance of the patient- appears to have a broken arm and road rash.</p><p>Responsiveness level- patient is awake and alert.</p><p>Chief complaint- broken arm and head injury. No apparent life threats. </p><p>Airway- open and patent no adjunct is necessary. </p><p>Breathing- good deep ventilations, no oxygen needed, no injuries compromise breathing.</p><p>circulation- good strong pulse, skin is pink, warm, dry, no major bleeding, patient is not in shock</p><p>priority patient- make this patient a priority with the head injury. </p><p><br></p><p><br></p>]]></description>

         <description><![CDATA[<p><strong>Secondary assessment</strong></p><p><br></p><p>Head-Hematoma to the front of his head, no obvious deformities to the skull, pupils are equal and reactive bilaterally. </p><p>neck- No JVD, Tracheal distention, No pain or tenderness of cervical spine, no deformities either</p><p>chest- no instability to chest wall, good chest rise and fall, breath sounds are good</p><p>abdomen/pelvis- no pain/tenderness or rigidity in all 4 quadrants, no obvious injuries, and no instability to the pelvis</p><p>Lower extremities- all motor, sensory, and distal circulatory functions are good, does have some road rash on both knees</p><p>upper extremities- motor, sensory, and distal circulatory function's are good. does have swelling and instability to left arm. </p><p>Posterior thorax, lumbar, buttocks- posterior thorax is good no deformities.</p><p><br></p><p>secondary injuries- wraps up the abrasions on the knees. </p><p><br></p><p>load the pt up in the ambulance and reassess the patient vitals. </p>]]></description>

         <description><![CDATA[<p><strong>Hospital call</strong></p><p><br/></p><p>This is Medic 1 can you hear me ok?  We are coming in with a 10 year old male with a chief complaint of a left broken arm and a hematoma on the front of the patient head. patient was riding his bike from a chicken when he hit into a mailbox. patient was not wearing a helmet and hit the front of his head on the mailbox. patient is not on blood thinners. unknown if loss of conciousness. patietn does not remember hitting his head. patient does have a cervical color on. no other deformities besides the abrasions on his knees. patient is awake and alert. we do have an IV started on the pt right AC. Vitals are as follows BP 130/80, hr 110, rr 20, spo2 98%, glucose 90, temp 98.6, GCS 15. we do have the arm splinted. We are enroute to your facility and are about 10 minutes out. anything else you need from us?</p>]]></description>
