Hi! I am back again with the records of the last week of my elective posting. I still can't believe it had already been a month. So, let's go to this week summary!

Week 4 Summary

Throughout this week, I joined the physiotherapist in-charge for morning ward rounds to attach cryotherapy machines, deliver walking aids and supports to the patients. Then at around 11am, I assisted suction procedures, then went for lunch break. At 2pm, I joined the rehabilitation therapy sessions and interacted with the patients while they did the exercises taught by the physiotherapists. Overall, I was able to participate more as I got the gist of the routine from my first week of attachment here. It was a fun experience and I enjoyed this week a lot.

1. Were the objectives of the project fulfilled?

   • Overall, I think all 3 of my objectives are fulfilled.

   • First, I get to see some of the non-pharmacological approach in patient management such as simply applying ice over swollen parts of body to reduce inflammation and repeating rehab exercises to prevent muscle atrophy.

   • Second, I get to join diet counselling sessions and observe first-hand how the dietitian word out her questions depending on the personality of the patient she is dealing with. I think this helps to ensure good communication and flow of counselling so that the messages delivered are clear and impactful.

   • Third is I managed to observe and even perform and assist in the procedures in physiotherapy such as chest physio, installing the cryotherapy machines and suctioning.

     
     

2. What I enjoyed or liked most?

   • The thing that I liked the most throughout the posting is learning how to communicate with staff, patients and their family members.

   • Personally, I think I am still lacking in the skill to find topics that I can use in conversation with patients to make them feel eased and more comfortable while doing the procedures. I came to realize that when the patients feel comfortable with the staff, they will let themselves be more vulnerable and voice out their health issues voluntarily.

   • Throughout my elective, I observed how different patient behave and take note on how the staff tackle these different challenges professionally.

     
     

3. What I found challenging?

   • Since I am attaching to a private hospital, I found it a bit challenging to actually perform some of the simple procedures such as chest physiotherapy myself because patients pay a lot of money and expect their care to be managed by professionals. I got to do a few but I think doing more certainly will help me to improve the techniques.

   • Nevertheless, I mostly practiced at home with my family members to grasp the skill better.

     
     

4. What has really worked?

   • I think in terms of knowledge, I definitely learnt a lot of new information and skills that are not taught in our syllabus.

   • In terms of experience, I got the opportunity to visit some of the hospital facilities such as the main kitchen, purchasing unit, pharmacy and physiotherapy equipment. I also got better and more confident in driving as I need to drive myself to the hospital everyday.

   • In terms of social skills, I learnt how to communicate with the staff at different departments, with patients of different age group and parents.

     
     

5. What can be improved?

   • Things that can be improved include more interactions with patients earlier. I was a bit anxious during the first few days and mostly just observed things. However, I wished I had been more friendly and just talk with them as they are nice and inviting if we treat them well.

To conclude, this one month of my elective posting is one of the most memorable period of my third year journey!

1. Were the objectives of the project fulfilled?

Yes, the objectives of my elective posting were largely fulfilled. My main goals were to gain hands-on experience in cardiology, improve my diagnostic skills, understand on how to read ECG interpretation and improve my history taking skills and physical examination skills in CVS on both paediatric and adult patients with the comments given by the professionals. I also aimed to familiarize myself with the hospital workflow and build professional relationships with doctors as I believe networking is very important in medical field. Through active participation in ward rounds, clinic attachments, attending CME & MDT meeting and independent learning, I achieved these goals.

2. What have you enjoyed or liked the most about the project?

I particularly enjoyed the hands-on experience in paediatric cardiology as it was both fun and challenging to work with kids, learning new things from the professionals and attending conference that was held at auditorium by the delegates on how to reduce cardiac cases and MDT meeting done at radiology department was a new experience for me which I really enjoyed too. The opportunity to be involved in a real-life cardiac case, like diagnosing acute coronary syndrome and presenting it to the senior cardiologist there was highly rewarding.The comments and advices he gave me after the presentation on the last day will be my lifetime motivation as it was very inspiring to me. Additionally, the warm and welcoming nature of both the paediatric cardiology and cardiology team made the experience enjoyable and motivating.

3. What have you found challenging about this project? (any part of the project you found hard to do)

One of the biggest challenges was clerking the parents in the paediatric ward because it was a new experience for me to clerk in pediatrics. The pain described by the parents might be not same as what the child is experiencing but i believe i did my best. Also, clerking patients without referring to their medical files was difficult at first, but it helped sharpen my diagnostic skills. Adjusting to a busy clinical environment also required mental focus to stay engaged all day. It was also quite difficult to adapt myself to the work environment there alone as I was the only one doing my elective posting at IJN from UKM. Hence, I'm forced to be independent in order to survive there.

4. What has really worked? (any part in the project that you thought worked well)

The patient interactions and clinical exposure worked very well for me. Dealing with the kids and their tantrums while getting history from the patients taught me so much of patience in being a doctor. Being asked to clerk a patient without relying on their medical history helped me grow in confidence and make decisions based on my clinical knowledge. The mentorship from the doctors, especially Dr. Fatin, also contributed to my positive experience. Hands on experience on patient by hearing prosthetic click sound from a patient with dual valve replacement was interesting. Attending conference and MDT meetings with the professionals was a great experience in my life.

5. What can be improved in this project?

An area for improvement could be better coordination between students and supervisors, especially during times when a supervisor is unavailable. More case presentation to the specialist could also enhance the learning experience as I only get to present cases a few times. Lastly, having more opportunities to observe and assist as in scrub in invasive procedures, like cardiac catheterizations, would provide additional learning.

Assalamualaikum and good evening, everyone! Today is the last day of my elective posting at KPJ Pasir Gudang Specialist Hospital. I would love to share some highlights from my last week, particularly my last day here. Happy reading!

Some of the exciting things I did during week 4 while attached to the dietitian, Puan Mun, included kitchen store inspection, inpatient and outpatient counselling in the ward and clinic, completing my website, and lending a helping hand during the medical checkup for the NSG company workers. The Google Form that I created during the third week was used at the medical check-up, and the data will be used by Puan Mun in her research on NSG workers' diet intake. I was delighted to help her in so many ways, even if it was a simple task like taking pictures of an unlabelled, unsealed container during the inspection. Without doubt, I have learnt a lot from her, especially during the consultation with the patients.

Here are some of the objectives of the project that I have fulfilled:

• Firstly, I have obtained a thorough understanding of nutritional management and dietary counselling, particularly for patients with diabetes, hypertension, and diabetes in pregnancy.

• Next, I have also enhanced my knowledge and skills in promoting physical well-being, reducing pain, and improving the quality of life of the patients I have attended with the physiotherapists.

• Lastly, throughout my weeks here at KPJ, I have managed to sharpen my communication skills to interact collaboratively with the dietitian, physiotherapists, hospital staff, as well as patients of all ages.

Things that I enjoyed/ liked the most about this project:

• I love the fact that now I am being able to examine things from multiple perspectives beyond my medical background. As a medical student, I occasionally failed to recognise that my decisions were entirely dependent on my medical expertise. However, after spending four weeks with dietitians and physiotherapists, I have come to respect other elements of healthcare, such as allied health.

• I am grateful to them for teaching me stuff I had only heard or seen on the internet. Performing chest percussion, learning about many types of procedures like cryotherapy, laser therapy, nasal suction, and the list goes on. Not only that, but I also gained a lot of dietary advice that I can start using right away to modify my uneven eating habits as a medical student who frequently has irregular meals. I can't wait to share these with my friends, as we constantly forget to eat healthy partly due to our hectic schedules, especially now that we're starting year 4.

The challenges that I found about this project:

• I think the top challenge I felt throughout this elective project was building a rapport and communicating with the physiotherapist and other staff workers. I am not quite sure if this is their culture, but the majority of the staff are super friendly and super extroverted. As an introvert myself, I find it hard to adapt at first. However, as time passed, I was able to cope and adapt slowly, and of course it all began with giving a bright smile to everyone I met in the wards, clinics, or elevators.

• During the first week, I struggled to adjust to the long working hours. However, because I go to and from the hospital with my friend every day, the tiredness fades when we spend time together and unwind after a long day. This makes me feel less alone, and we always cheer each other up to get us through the day.

Things that have worked well throughout this elective posting:

• Firstly, I have learnt a lot in terms of the physiotherapy and dietary knowledge. I am grateful to the staff who always teach me and guide me to perform procedures I have never tried before. I hope I can apply this knowledge and skills in the future to explain to my patients if they are in doubt.

• Moreover, I have also upgraded my social skills as I continue each day talking and communicating with the KPJ staff, my supervisors, and the patients from all walks of life. Hopefully, this experience will boost my communication skills to be applied during my postings in year 4 later.

What can be improved in this project?

• One area for improvement in this project is my confidence in initiating conversations with new people, which I battled with during my initial days at KPJ. I am confident that with some practice, I will be able to master these abilities and be a more approachable person to my patients and hospital staff.

To sum up, I had spent an amazing 4 weeks here at KPJ Pasir Gudang Specialist Hospital, and I felt valuable as a human being because I was able to contribute some things even though there were not that many to my physiotherapist and dietitian that I worked with. I would surely remember this place and all the nice people I have met during my elective here, as this has been quite a memorable experience in my life so far.

Were the objectives of the project fulfilled?

After going through 4 weeks of attachment in Klinik Pergigian Noorhani, my objectives for this elective posting have been fulfilled successfully.

I managed to improve my communication skills, especially with patients and co-workers. My communication with them got smoother on the 4th week as compared to first week. It might seem simple but I realised I needed to adapt to the situation and age group to ensure effective communication with patients. This required skills and practices.

Besides, I managed to observe quite a number of dental procedures and learnt basics dental treatments. This included teeth extraction, filling, scaling and polishing as well as dentures. These procedures were among the common ones that I got to observe everyday for the past 4 weeks.

Lastly, I got to learn the usage of common dental instruments. It was quite hard at first as everything was new to me but I am grateful to the dentist and dental assistants for their help.

What have you enjoyed or liked the most about the project?

I think the thing I liked the most was dental making procedure. It was very interesting to me as I got to have hands-on experience instead of just learning the theory. It was indeed a very precious knowledge that I would treasure. Preparing the arginate with correct consistency was quite hard as there are times that it became too watery but with the help of dental assistants, I managed to improve on that.

This same goes to preparing the fillings. Everything needed skills and I think I manage to at least got the basics of them.

What have you found challenging about this project? (any part of the project you found hard to do)

I think the challenging part of this project was communicating with the patients especially elderly and foreigners. I needed to use simple explanation to the elderly to ensure they understood what I was conveying well, this included instructions to take care of their dentures. As for foreigners, some of them could not even understand both Bahasa Melayu and English, making it hard for me to communicate with them. I needed to speak slowly and find other alternatives to convey the information needed. Things were easier with the help of the dentist and dental assistants as they were already used to this situation.

Apart from that, I found it quite difficult to memorize the name of each instrument and the usage of them. I needed to refer to my notes again and again until I felt more familiar with them.

What has really worked? (any part in the project that you thought worked well)

One of the things that has worked for me was my soft skill has gotten better. I got to improve my verbal and non-verbal skill when communicating with patients and the others. This would be useful for me in the future as I would need to deal with a lot people when I started working.

Besides, I got to increase my knowledge regarding the basic of dentistry. It might not be in detailed but I felt that they were sufficient and quite a lot, given the short time of attachment I had.

What can be improved in this project?

One thing that can be improved in this project is the opportunity to perform basic dental procedures such as scaling and polishing. I can understand why I am never given a chance to do that as I do not formally learn dentistry and the dentist is probably afraid that things might go sideways. However, I think it would be a more fun experience for be if I were given the chance to do it myself with supervision.

Personally, I think most of my objectives were fulfilled, in terms of communication with patients, learning dental skills, and experiencing dentistry life which totally different form medical.

In terms of communication with patients, I actively interacted with patients, especially children, helping them stay calm and comfortable during procedures. Next, I also have assisted in procedures like suctioning and observed several dental treatments. I have also been involved in tasks like sterilization, learning practical dental skills. Basically, I was actually been immersed in the day-to-day environment of a dental clinic, from patient interaction to understanding the clinic’s workflow. This helped me compare and contrast with our teaching hospital environment.

   - Hands-on experience with dental tools and observing various procedures like root canal treatments, which captured my attention.

   - Learning about the importance of sterilization and how much effort goes into patient safety behind the scenes.

   - Building rapport with clinic staff and patients, especially interacting with children and helping them relax.

3. What have you found challenging about this project?

   - I experienced some difficulty in understanding dental terminology initally, however I’ve taken steps to familiarize myself with thise terms by taking notes.

   - Handling pediatric patients especially managing an upset child during a procedure was a challenging experience, requiring me to stay calm under pressure, especially when the parents accompanying the child were also anxious as well.

   - Standing for long periods during the day has been tiring, especially the procedures and treatment were repetitive.

4. What has really worked?

   - In terms of observation and participation, I have taken an active role in observing treatments, assisting with suction, and learning about sterilization, hence it has worked well in helping me grasp the practical side of dentistry.

   - In patient communication, I managed to engage and communicate with patients and it has helped to build trust and keep patients at ease, especially during potentially distressing situations.

• Going to the dental clinic and observing the procedures everyday has helped me grasp more knowledge about dental hygiene and care better than I usually knew before, and I can help to educate other people to take a better care of their dental hygiene form the knowledge I gained in the clinic.

5. What can be improved in this project?

   - Deeper involvement in procedures: I could have discuss with the dentists about getting involved in more advanced tasks or procedures to deepen your practical learning experience.

   - More structured learning of dental terminology: Setting aside time specifically for learning terminology or asking the dentists to explain terms during or after procedures could help you grasp them more quickly.

   - Taking more initiative: I should ask for more opportunities to participate in simple tasks that could accelerate my learning curve and make me feel more involved in the process.

During my four-week elective posting at the dental clinic, I believe the objectives I set for the project were largely fulfilled. Firstly, I had the opportunity to observe and learn effective communication strategies used by dentists in their interactions with patients. I noticed how they explained procedures, addressed concerns, and built rapport, which deepened my understanding of patient-centered communication. Secondly, I gained significant exposure to the function and usage of various common dental instruments and technologies, which enhanced my knowledge in this area. Lastly, I was fortunate to assist dentists during certain procedures under supervision, allowing me to acquire practical skills and gain firsthand insights into clinical practice. Overall, the elective experience was instrumental in meeting my learning goals.

What I enjoyed the most about my elective posting in the dental clinic was the chance to assist dentists in various procedures. I also made new friends during the posting, among the dental team, which made the experience more enjoyable. Additionally, assisting during dental procedures was initially difficult, as it required coordination and a detailed understanding of the process, something I hadn’t encountered in my medical training. Adapting to the pace and expectations in a dental setting was a learning curve, but it ultimately enhanced my skills and broadened my experience.

4. What has really worked? (any part in the project that you thought worked well)

One aspect of the project that really worked well was the opportunity to observe and assist during dental procedures under supervision. This hands-on experience was invaluable in helping me understand the practical side of dentistry, which complemented the theoretical knowledge I gained. The structured guidance from the dentists, combined with the chance to directly apply what I observed, allowed me to feel more involved and engaged. Additionally, the communication between the dental team and myself was seamless, making it easy to ask questions and receive feedback, which greatly enhanced my learning experience. 5. What can be improved in this project?

One area where the project could be improved is by offering more structured opportunities for medical students to participate in hands-on procedures. While I had the chance to assist in some dental treatments, expanding the scope of tasks for students could enhance practical learning. Additionally, more formal teaching sessions or workshops focused on dental anatomy, common procedures, and the use of specific instruments would be beneficial for students unfamiliar with the field. Lastly, incorporating a clearer schedule of activities at the start of the elective would help students better plan their learning goals and maximize their experience throughout the posting.

Week 4: 26 - 29th August 2024

Hi Prof. Farah and Group D mates, it's nice to see the last week of your electives already uploaded here. I'm glad to have seen your cumulative experiences from the past month of our elective. Sorry for the long wait - after a quick escape from the hectic first few weeks of my paediatric posting, here I am to share my last week of elective with you all. I truly loved and cherished that time and just wish I could turn back the clock to relive those wonderful moments.

Were the objectives of the project fulfilled?

Prior to the elective project, in my elective form, I had listed three objectives I wished to fulfill. Firstly, it was to deepen my understanding of other roles besides doctors in the healthcare system, specifically that of a clinic assistant. Needless to say, I have come to understand the pivotal roles they play in the clinic, working alongside them. I would say their contribution is crucial in keeping the clinic running swiftly and smoothly. From setting up the clinic in the morning, registering patients, arranging insurance, preparing and assisting doctors in procedures, handling prescriptions and labelling medications, to calculating costs for self-pay patients - they do most of the tasks in the clinic. Through this experience, I have gained immense appreciation and respect for how efficiently they manage all of these responsibilities.

Secondly, my next objective was to strengthen my communication skills, build confidence in my abilities and cultivate a mindset of continuous learning and improvement in the healthcare domain. Being with Dr. Wan and Dr. Meera, I learned a lot about effective communication and building confidence, as I mentioned in my previous writings on the padlet. They were always willing to teach me, and although I hadn’t yet experienced the obstetrics and gynaecology posting, I was able to start from zero and gain substantial knowledge during this elective. This experience even piqued my interest in exploring the specialty further in the future. Dr. Wan would often give me the opportunity to explain scan findings to patients, discuss their conditions, and even take a few patient histories during the last week of my elective. These hands-on experiences were invaluable in building my communication skills and boosting my confidence in patient interaction.

Third and last but not least, my final objective was to develop empathy and gain a deeper understanding of a patient's needs, concerns, and experiences through interactions in the clinic. Reflecting on my journey, I can confidently say that compared to before joining the programme, I now view patients from a different perspective - one that is rooted in empathy. Patients may come to the clinic solely for their illness or condition, but their experience is often much more complex. By interacting with them, I’ve come to realise that beyond their medical issues, there are emotional and psychological concerns that need to be addressed. This understanding has allowed me to appreciate the importance of approaching each patient with compassion and a genuine desire to help them through their health challenges. I would always remember Dr. Wan’s advice was to always let the patient talk, not to cut them off. They need to be heard for you to know what problems they are facing and help in finding a solution to it and advice, medically.

In a nutshell, I’m thrilled to say that all three objectives of my elective were not only fulfilled but actually way exceeded my expectations. Each goal, from gaining a deeper appreciation of different healthcare roles, to sharpening my communication skills, and finally, fostering empathy towards patients, was met with invaluable hands-on experiences and personal growth. The four weeks have been a transformative journey, allowing me to walk away with new perspectives, greater confidence and a renewed passion for my future in medicine.

What have you enjoyed or liked most about the project?

I truly enjoyed the elective project, but the highlight for me was when I was given the chance to assist doctors with procedures. Being directly involved in the clinical process, from preparing instruments to observing and even lending a hand during certain tasks, was surely eye-opening. From simple procedures like taking blood pressure and weight, to more involved tasks such as surgical wound dressings, ear wax suction and irrigation, each experience added to my learning. The one I remember most vividly was scanning a pregnant mother and measuring the baby’s heartbeat on my own. It was an unforgettable moment, as I got to witness the miracle of life up close. These hands-on opportunities, no matter how small or routine they may seem, gave me a deeper insight into patient care and made my elective experience all the more rewarding.

What have you found challenging about this project? (any part in the project you found hard to do?)

One of the most challenging aspects of this project was explaining scan findings to patients, especially when dealing with conditions like endometriosis. Instead of diving straight into the medical details, I learned that it’s crucial to first reassure the patient that it is a benign condition. This approach helps to ease their anxiety and prevents them from becoming overwhelmed by fear. By providing reassurance early on, the patient can calm down, process the information and focus on what you’re explaining. Balancing the delivery of medical facts while maintaining a compassionate and calming tone was a delicate but important skill I worked on throughout the elective. 

I would like to extend my heartfelt thanks to Dr. Wan for guiding me on how to effectively explain and relay news to patients. Her support and insights were invaluable in helping me navigate these sensitive conversations. I truly appreciate the emphasis she has placed on empathy and reassurance, making a significant difference in my approach and confidence during patient communication.

What has really worked? (any part in the project that you thought worked well)

What really worked well during the project was the hands-on approach to learning. Engaging directly with patients, participating in various procedures, assisting with preparation of medications allowed me to apply theoretical knowledge in real-world settings. This practical experience not only solidified my understanding but also enhanced my overall communication skills and gave my confidence a boost. 

Additionally, the supportive and caring environment created by the amazing clinic assistants (Kak Madu, Kak Zila and Kak Tasha), doctors (Dr. Wan, Dr. Meera and Dr. Aslam) and  sonographer (Kak Thania) fostered open discussions and feedback, which encouraged me to grow and refine my approach to patient care and how to work well with a team of interprofessionals, which remind me bits of the IPL we had in Year 3 . Overall, this blend of practical involvement and mentorship proved to be highly effective in my learning journey ahead.

What can be improved in this project?

I cannot think of any particular parts I wish to or can be improved in the project. Overall, the experience was totally enriching and I feel that it effectively met my learning objectives.
