The presence of ribosomes is the primary distinction between Rough Endoplasmic Reticulum and Smooth Endoplasmic Reticulum. Rough ER gets its name from the distinctively rough appearance it takes on when ribosomes connect to its surface. A smooth ER, on the other hand, doesn't have ribosomes on its surface.
