CalWORKS Case Management Services

At each of the six CalWORKs sites in San Diego County, Alcohol and Drug (AOD) Specialists screen clients for substance abuse and mental health issues that may affect their ability to work. If needed, they refer clients to appropriate mental health services and connect them with substance abuse treatment. Family Stabilization (FS) Specialists also provide short-term case management to help clients overcome challenges, offering support through home or work visits and access to resources like housing, health, and transportation assistance.

Who is Eligible?

The Vista Hill CalWORKs Case Management Program works with adults that are involved with the CalWORKs Welfare to Work Program.

The ACT program services include specialty behavioral health and psychosocial rehabilitation services, substance use treatment, nursing, psychiatry, and medication management. Individuals also receive housing and housing assistance, assistance with benefits and entitlements, education and employment services, care coordination, and assistance with activities of daily living.

Adults 18-59 (Senior IMPACT for ages 60 and older).
Must reside in the corresponding catchment area.
Have a serious mental illness with functional impairments such as inability to maintain housing, employment, or education. Individuals with co-occurring substance use conditions in addition to a primary serious mental illness are welcome.
Experience homelessness or at-risk of homelessness.
Deemed to require ACT level services based on the Level of Care Utilization Scale (LOCUS).
Voluntary participation or have a conservator willing to sign consent for services.
Medi-Cal, Medi-Cal eligible or uninsured accepted. No one is denied services based on inability to pay.
