         <description><![CDATA[<div>From my own points of view,Nurse Smith did not apply the principles of record keeping in this scenario,if she applies a good record keeping she would have handed the record to the student nurse to use when dispensing the drug,even though a student nurse should not be left alone without proper supervision by a Registered Nurse when carrying out any procedure.Nurse smith was only seen to report the incidence but adequate record was not kept.<br>Also,she delegated a task to the student nurse to monitor the patients vital sign which she did not do,in my own opinion the student nurse that has commited such an acts should not have been delegated to carry out vital sign,the same student was sent on her break by who?if i may ask.Is it the same nurse smith or another nurse that is not aware that a task has been assigned to her or who?all this are claims of negligence that the patient has the right to hold the nurse,or even the hospital accountable for litigation.</div>]]></description>

         <description><![CDATA[<div><em>It is clear that in the scenario, nurse smith did not adhere to the principle of proper record keeping. Her lack of this may have been as a result of, according to her, being short staffed and having a busy shift.  According to the reply, there were no first time observations by which the doctor could make a comparison.  She felt that it wasn’t her fault and student should have known how to administer the medication or record patient’s condition. The lack of this record keeping caused a delay in treatment of the patient.  It is also a reason for litigation against professional and institution. <br></em><br></div>]]></description>

         <description><![CDATA[<div>Nurse Smith did not apply the principles of good record keeping, and she failed to take accountability for poor delegation of someone who failed to carry out the delegated task. She should have been aware of the student's capabilities or lack thereof by ensuring that the students are always supervised when carrying out tasks. It does not state if she is the one who admitted Mr Black, but upon noting the absence of baseline observations, her duty was to alert those responsible to ensure that all the patient's details were entered in his notes, this in turn delayed the patient's treatment. She then went on to delegate the same student who had previously failed to carry out her task, due for her break and still left her unsupervised.<br>Nurse Smith was supposed to ensure that patient safety and care is not compromised by reporting the staff shortage and requesting support staff on the busy day, it does not seem like she did.</div>]]></description>

         <description><![CDATA[<div>Principles of record keeping<br><br>Based on the scenario it is evident that Nurse Smith did not apply the principles of record keeping. For example there is no documentation to indicate accountability for her actions, to support delivery of service and to detect the possibility of complications. Despite being short of staff, she should have been aware of the limitations of her available staff and seek for assistance before,  from her superior's before  attempting to handle the ward on her own.  Further more, the student made an error in administering the wrong medication to the wrong patient because of lack of supervision on her part, but yet that same student was given the responsibility to monitor the patient. Lack of communication is definitely present, because there is no records of the vital signs and the student was sent out on break, leaving that same patient unobserved. There is no documentation to indicate that it was clearly explained to the student , should have assessed and recorded the vitals and observe the patient for a certain period of time. Also based on Nurse's Smith statement, she assumed that the student was capable of administering the medication and overseeing the patient alone  , because of being a second year student. The student should have been assessed before the delegation of such responsibility.  Finally,  Nurse Smith took no accountability for her actions, but attempted to place blame everywhere else. </div>]]></description>

         <description><![CDATA[<div><strong>It is evident in the above scenario that Nurse smith did not apply the principles of record keeping. She was obviously too busy because they were short staffed. She should have given the record to the student to administer the medication correctly to the patient and the student nurse shouldn’t have been left alone with the medication. Also the student nurse was not asked to administer the medication, there was certainly a misunderstanding. Nurse  smith wasn’t accountable for her actions, Proper documentation did not take place on admission, the nurse was not also empathetic about the mistake that was made but was trying to justify it. Nurse Smith should have also told the management that they were short staffed. Mr James Black blood pressure also became elevated probably because the wrong medication given to him. The student nurse was only sent on a break, she wasn’t told about the gravity of the mistake she had made and also she didn’t observe the patient and his vitals as she was asked to. <br>I commend the fact that Nurse smith immediately reported the error to the doctor. <br>Lastly the claim of negligence can be made.<br></strong><br></div>]]></description>
