The Schuylkill County Transition Council (SCTC) meets quarterly to promote effective transition for students with disabilities in Schuylkill County. The council is comprised of parents, agencies, district transition coordinators, business representatives, and educators. The SCTC shares information from the State Interagency Advisory Council on Transition.

Pennsylvania educators facilitate students’ successful transition by using a six-step process to develop the IEP; guide the way for students, families, educators, and service providers; and prepare students to cross the “bridge” to adult life.

Supporting a child with a disability through their education takes a team approach. As a parent, you are a key member of your child’s educational team. PaTTAN’s Family Support Specialists and other staff aid families in understanding the special education process, share information and resources, and work with state agencies who support families like yours. PaTTAN encourages partnerships among families, schools, and communities to foster success for all children.

Healthcare Transition

Health care transition, or HCT, is the process of getting ready for health care as an adult.

During childhood, your parents and caregivers usually help with your health and health care needs—they call for appointments, fill out forms, and keep track of medications. As you get older, managing those needs becomes your own responsibility. Achieving this independence requires an organized transition process to gain independent health care skills, prepare for an adult model of care, and transfer to new clinicians.
