If  I think it is major enough that they may need to be on an improvement plan in the future I share that with them as well. However, I make sure to give them time to improve and get support. I also reach out to the mentor teacher to hear their thoughts and also offer them support if needed.

This can help them feel more comfortable (and less defensive) and help them recognize the moves we are talking about are all about helping students.

Ask what they feel they need the most support with. Reach out to the mentor and see if there are any concerns that they are seeing. Sometimes the candidates can be too critical of themselves, so you have to put a lot into perspective for them.
