         <description><![CDATA[<p>MHM-CONREP (Conditional Release Program)</p><ol><li><p>Strengths: clearly stated mission on their website (kind of); attempt to assist clients with re-entry into society following incarceration/state hospitalization; provide case management services to clients who otherwise would not have access to them; current director attempts to prioritize staff well-being</p></li><li><p>Weaknesses: treatment sometimes feels punitive versus rehabilitative; power rests with providers; high clinician turnover ; community members are not involved in decision-making </p></li><li><p>Opportunities: policy updates (all policies are DSH, so many are old and outdated); increased education/training</p></li><li><p>Threats: decreased funding would harm the organization; punitive actions can create harm to clients through decreased sense of autonomy; power creates subjective decisions that can lead to re-incarceration </p></li></ol>]]></description>

There is a variety of teams focused on different cultures, ages, and justice impacted clients.</p><p><br/></p><p>Weakness: Some policies prevent continuity of care between teams or clinics. </p><p><br/></p><p>Opportunity: Cross training would provide insight and flexibility among staff and foster a more supportive experience for the clients. </p><p><br/></p><p>Threats: Only serve Medi-Cal recipients.          <description><![CDATA[<p>Community Medical Centers</p><p><br/></p><p>Strengths: They employ many providers who are representative of the community that they serve. The main mission of the organization is to provide “healthcare for all” and believe that “everyone in our community deserves quality healthcare delivered with dignity and respect”</p><p><br/></p><p>Weaknesses: Their hands are tied over who is eligible to receive care depending if clients have MediCare/MediCal. They are also very backed up as far as appointments go and people may only be seen once once every 2 months at the soonest.</p><p><br/></p><p>Opportunities: They have a current partnership with the city of Stockton which resulted in a mobile community response team which can respond to cases which involve mental health crisis where police can’t really help. </p><p><br/></p><p>Threats: services are only available as long as funding is available and if documentation isn’t done then the organization does not get paid. </p>]]></description>
