GPs agree that the patient record and information system needs to be changed in a way that meets the organisation's record-keeping and information management system requirements.<br><br>Solution: Transition from paper-based records to an electronic system.<br><br>Why it should be changed:<br>*Cost- Using electronic medical records (EMR) is less costly over time, including physical storage space, time and people needed to organise documents.<br>*Storage- Much less space used; more private and secure.<br>*Security- Keeping records on a reliable electronic system is more secure, as paper records can get lost, damaged, or stolen.<br>*Access- Processing and recovering paper records is time-consuming, especially when being transferred between healthcare professionals or locations. EMR provide access to information almost instantly.<br>*Readability and accuracy: Paper records can be difficult to read due to untidy handwriting, and may not have enough space for important information.          <description><![CDATA[<div><strong>Action Plan </strong><br>- Propose the Plan to the relevant people ( Paper based records to electronic record system )<br>- Conduct a meeting or conference with the people who will be involve. Present the project's description, objectives, rationale , advantages etc.<br>- Provide allotted time to conduct surveys, comments, suggestions from the relevant people.&nbsp;<br>- Once the project has been approved, it must be implemented, Be sure that everyone has been advised early or months before the changes will occur.<br>- Conduct conference and invite an IT specialist or Person who is professional in using the System to discuss, present&nbsp; or instruct the relevant people especially the Administration Staffs on how the operating system or PracSoft is being used).<br>- When the system is ready to be used and the staffs are well-informed and trained, All the paper based records must be transferred, saved and secured in the Electronic System. And all the paper based records must be kept safe and secure for future references.<br>- After weeks that the electronic system was used, seek feedbacks, suggestions from the staffs.          <title>Question 7. Action plan to implement the new/improved work practice.</title>

         <description><![CDATA[<div>*Identify issues that may affect the implementation of the change, eg. cost of the software and computers/IT system, training required, staff resistance to the change.<br>*Once possible solutions have been considered, ideas for how the new system could be implemented should be presented to management. Presentation of the idea should involve benefits to employees, stakeholders, and the organisation, as well as constraints and possible solutions.<br>*Support and feedback from other staff member should also be sought, eg. through meetings where all staff (including management) are present, and managers who are involved in the change can keep staff up to date on progress. Staff can also voice their concerns and provide feedback.<br>*Once funds have been allocated, purchase computers and Pracsoft software. Hire IT staff to train other staff in using the program, including how to transfer information from paper records to the electronic system.<br>*Feedback from staff should be continually sought throughout the transition from paper to electronic records. Management can gauge if the change is an improvement, and if not, what other adjustments can be made to ensure that staff are knowledgeable and confident enough to continue using the electronic system.<br><br>-Stephanie Murray</div>]]></description>
