Trimethylaminuria (TMAU) is an extremely rare genetic disorder which causes you to have a very distinct smell, usually of a fish. This disorder is also commonly known as fish odor syndrome or stale fish syndrome.

TMAU was first discovered in 1970 by J.R. Humbert. Humbert first came upon it when a 6 year old girl and her mother came to him complaining of the girl having a fishy smell. There have been very few cases since (About 200).

TMAU is a disorder that is inherited from your parents. It is when you have an impaired version of the enzyme flavin with monooxygenase 3 (FMO3). It is an autosomal recessive gene disorder where both parents carry one altered FMO3 gene. There is a variable in the symptoms of getting the disease which can be from mutations in FMO3. This can affect how strong the smell is and the time you get it. It can also be triggered by stress and diet.

TMAU is usually tested by a urine test. If your urine is found to have high trimethylamine levels, then you could be diagnosed with the disorder. The test is a ratio from trimethylamine to trimethylamine N oxide in the urine.

TMAU has a negative impact on the victim's daily life. Some examples are low self esteem, getting a job, and unfortunately attempting suicide. This disorder is much more common in women then in men. Scientists think that it is from the specific hormones that women have like estrogen and progesterone. Some cases have been reported to be stronger during puberty.

There is no specific way of treating this disorder, however, you can decrease the intensity of the symptoms by avoiding foods with high levels of trimethylamine and its precursors. Also to decrease symtoms, a person can take certain supplements.
