It looks too small for the big bottles I am designing for, and it also needs a larger surface area, but I bet this would be cheap to print. I imagine that to actually use it you would simply grip the whole top & gadget together in the right-hand and push. I like the way this allows the user to push against the bottle-top with the shoulder and arm, rather than just using the fingers and wrist to unwind it.

It would need to be much bigger to be able to spread the force over the fingers though.
