The revised email does not require you to write the students name amongst the email, nor are you required to address the email using the MT’s name, (however if you wish to address the MT you may).  
 
You will need to fill in your name and email address only,  you have saved a copy you can just cut and paste the same email and send it. If you have any questions, please let me know.
