The stark juxtaposition Kesey establishes between the identities of the Big Nurse and McMurphy allows for the main struggle of vying for power to be introduced; by illustrating each through their different levels and sources of dominance, he sets up the tension between the two that will be seen throughout the book.
Within authority, there is power, however within power there is dominance. When Kesey writes "I know it's the Big Nurse by the way the lockworks cleave to the key" this exemplifies how much power she holds over them; she can cut them down so they fall around her feet like fallen raindrops, yet they need her still.

Though their authority and dominance is derived from their jarringly different personalities the tension still remains.

The automatic authority Big Nurse is given is displayed by her physical description of being "smooth, calculated and precision made" showing she makes not mistakes.

By juxtaposing precision and unruliness, machinery and nature in Big Nurse and McMurphy, Kesey constructs a dichotomy of character through which the main themes of oppression and individualism are exposed.
From the initial introduction of Big Nurse, Kesey establishes her as a domineering presence, calm and cold. "With a gust of cold" Big Nurse occupies the passage, exuding authority and machine-like presence.

Kesey contrasts Big Nurse, associating her with unnatural sterility and properness, with McMurphy, describing him with a natural, wild rawness, to create the idea that there is more kindness and humanity present within graceless chaos than there is with overbearing authority. Big Nurse's first movements are characterized by likening her hand gesture to clean metal, imagery that emphasizes the disturbingly impenetrable authority that she holds: "...and I see her fingers trail across the polished steel."

The connotation that McMurphy carries as representing natural disorder juxtaposed with the implications that Big Nurse carries as a symbol for forced order therefore suggest that their is a lack of any real humanity within artificial law, and the more humane aspects like comfortableness and homeliness lie ironically within wild chaos.
