NPD's direct cause is unknown, but there are many risk factors. It can stem from insensitive parenting, abuse, over-praising from parents, negligent care, excessive criticism, trauma, or extremely high expectations. Other possible factors are physiological such as abnormal genes that affect the connection between the brain and behavior.

Narcissism affects more males than females and a lot of times it begins in the teens or early adulthood. About 6.2% of the US population has NPD. It's pretty evenly distributed among races. NPD was significantly more prevalent among younger adults, and separated/divorced/widowed and never married adults. NPD was associated with mental disability among men but not women.

“I have low self-esteem and low self-image stemming from not being given adequate (or being given too much irrational) attention from my parents as a child. I have, as a defence mechanism, cultivated an artificial persona that makes me look more intelligent and more important than I really am — and I need YOU, my lowly co-dependent, to provide me the validation I need so I can feel better about myself. And I ALWAYS need to feel better about myself. If you don’t give me the validation I need, I will coldly manipulate and gaslight it out of you.”

“Narcissus weeps to find that his Image does not return his love.” ~ Mason Cooley
