I used this same format here 2 years ago with the seniors. Regarding the math goal, we had to have a 3-tiered intervention that was SMART goal format that matched the school's overall goals. Teachers and staff provided the academic interventions and my counselor partner and I implemented our three tier intervention. The three tiers of intervention used were: contracts filled out and signed by parents was delivered in whole-class format, parent phone calls and conferences were used with a smaller group of the lesser motivated students, and for the very lowest tier of students, we created a "Why Try" small group to work on motivation. We will use Google Classroom to send each student their own copy that will be shared with all teachers. It will be used to set and track goals. We recognize this is our first draft and use of this template.
