a. A brief synopsis of what occurred. The challenger disaster was a space shuttle that after launched in January 1986 it blew up and lost the crew and was spread nationwide. they believe that NASA was to blame for it.

b. Key individuals (who they are, relevant background information, role in the event) There was a former secretary of state William P. Roger who had the so called Rogers Commission to gather all of the data and each piece of the shuttle that they could recover to try and figure out exactly what had happened. As the article states the source of the disaster was a O ring that can cause a numerous amount of problems with the shuttle.

a. Symptoms of Groupthink (identify 3-4 different symptoms and provide examples to support your answer) According to the book article Janis originally defined groupthink as "a mode of thinking that people engage in when they are deeply involved in a cohesive in-group, when the members' strivings for unanimity override their motivation to realistically appraise alternative courses of action."5 According to his definition, groupthink occurs only when cohesiveness is high

b. Effects of Groupthink in this event (provide 2-3 specific examples) that group could have all come up with a cohesive decision on the faultiness of the shuttle rather than blaming NASA

c. How Groupthink could have been prevented in this situation
Collective Rationalization, Direct pressure of dissenters. As well as Self appointed mind guard, illusion of unanimity,
