Since they now use electronic forms patients don't get anything. 
I think a card that crews could leave the patients with details of the complaints, compliments contact, names of crew, job number short treatment summary and any safety betting advice/ signposting. 
Currently there is a whole team of people who spend their whole day trying to track down jobs and crews to pass on a compliment be ause the atient never have a job number.
