However, there still is some and often times short clinical and professional language when dealing with every form and chart, dental hygienists have the patient fill out the normal questions dealing with the patient's name along with their dentist. The form needs their date of birth, address, phone number, and basic factors as mentioned. All typical moves serve a purpose for both patient and their dentist/ dental hygienist. Some factor that could go wrong is performing the wrong treatment or surgery on a patient. Not writing down the correct email or phone number will be an issue to get ahold of the patient. Obtain patients' written consent before disclosing any of their health information like electronic records, for a certain period of time. Notify patients of their rights under HIPAA, such as the right to access and amend their health information. Report any breaches of protected health information to patients and the U.S. Department of Health and Human Services.
