Misuse of company twitter account, photograph taken by a member of staff who unknowingly captured the personal details of a client in the background.  This breached GDPR regulations and also internal policies of use of social media.  

Consequences
Photo removed from twitter, resulted in reputational risk to the organisation and breach in data protection. Internal investigations identified that the member of staff did not have to go through the disciplinary process nor be reported to the ICO.
