Suicide risk assessment: High
 - Suicide ideation since 13 July 2019. Student continues to have thoughts about suicide.
- Two prior attempts in 2020 (March and August) but were stopped by family members.
- Diagnosed with Major Depressive Disorder (MDD) in September 2020. History of family members with similar mental health issues.
- Still affected by the news of online friend committed suicide in December 2020. 

Risk factors:
- Non-compliant with medication for MDD
- low motivation and self-esteem
- possible relationship issues with peers 
- relationship issues with parents, they disapproved her educational pathway

Protective factors:
- Two close friends that she can confide in
- Good relationship with maternal grandparents
- Motivated to complete "O" level examinations
- Still receptive to seeking treatment for MDD

Safety and support plan:
- Home support: Close monitoring by maternal grandparents and domestic helper 
- School support: By teachers, SC and peers to provide emotional support
- External support: Work with mental health professionals

Student had 2 previous suicidal attempts in 2020. Trigger : 1)diagnosed with  depression disorder in Sep 2020.  
2) Student has disagreement with her parents over her educational pathway.

Protective Factors: 
1) student is close to her maternal grandparents 
2) close to classmates
3) supported by ESU worker/SC
4)regular school attendance
SC kept in close contact with maternal grandparents/domestic helper to monitor students' mood. Subject teachers have been informed to accomodate student's academic progress.

Suicide risk is assessed to be high due to her self-harm behaviour (July 2019), MDD (diagnosed Sep 2020) and previous 2 attempts (2020). The triggers include online friend's death by suicide and ongoing family conflicts such as disapproval of her educational pathway. 

Protective factors include 2 close friends to confined in, mgm, uncle, sister and domestic helper. She is also motivated to complete o levels and receptive to treatment for MDD and counselling.

Counselling sessions will be twice  weekly. Close collaboration with mgm, helper, teachers, identified classmates(as buddies), mental health professionals, ecg counsellors and parents.

CoV (2 friends) was indentified and counselled and parents were informed.

Risk factor: history of self harm, facing parents disapproval of education pathway. Diagnosed with MDD in Spet 2020 and was not consistent in follow up with therapy session. And not compliant with medicine. Also online friend committed suicide. Hence case assessed to be high risk. 

Protective factor:
- good relationship with grandmother
- 2 close friends
- desire to complete O levels
- Receptive to MDD treatment

Interventions:
- regular check in with student during MC and student observed to be calm and cheerful
- brief teachers to support student
- advised  parents to restrict child's access to medication and alcohol. 
- teachers identify buddy as peer support

Follow up:
- ECG counselor to be consulted to work with student and parents
- liase with stakeholders for further intervention/ support plan
- SC will regularly see student for closer monitoring
