Clinicians work in an environment where slight deviations could cost a patient’s life; unfortunately, most teaching institutions still utilize educational materials available in two-dimensional (2D) format, and most clinical settings still lack the healthcare technology to enhance practice and ensure that the overall process performs at an exceptional level. The discovery of Microsoft HoloLens Augmented Reality and its programmable software tailors to various industries, companies, and organizations. The HoloLens is the first untethered holographic computer in the world, delivering three-dimensional (3D) imaging while using the latest optics and sensors to individuals and group users. The HoloLens allows users to create holograms, which are objects that mimic the ones seen in our environment. The projected objects are made of light and sound and can respond to virtual touch, gaze, gestures, and voice commands (Folsom, Zeller, & Bray, 2018, para. 1).

HoloLens hardware details: The HoloLens is a wearable device, where users secure the optics on the face, similar to wearing eye goggles. The device has see-through holographic lenses that have automatic pupillary distance calibration, built-in microphone, speaker, and is built using Intel 32 processor. The device provides 64GB Flash and 2GB RAM power. With all the features included, overall weight is only 579g (Zeller & Bray, 2018, para. 1).

HoloLens software app: The HoloLens is useful in various industries due to its ability to mix the user’s current environment with the 3D digital images, enhancing the worker’s environment. “This approach also allows users to easily move from real-world tasks to digital tasks and back easily, lending even more credence to promise that the mixed reality apps the user sees are truly a part of their environment” (Zeller & Bray, 2018, para. 2). Many clinicians, students, and educators have enjoyed the benefits of HoloLens use in academia and the operating room.

HoloLens in Academia:
Teaching institutions, such as the Case Western Reserve School of Medicine, have incorporated Microsoft HoloLens to enhance teaching. Teachers are able to present organs and several anatomical objects in 3D, with streaming video and graphics, where learners are able to see how a specific organ look under different environmental controls. By using HoloLens in medical academia, learners may have a better understanding of the structures and physiological processes that are happening in the body (Hololens for Medical Education, 2015, para. 1). After medical and nursing school, clinicians work in live environments where multiple patient events are occurring at the same time, affecting the outcome. The use of interactive 3D technology in academia will prepare students better for post-licensure practice.

HoloLens in clinical practice: The use of HoloLens in clinical practice is also gaining popularity, as hospitals adopt this technology in various settings. A well-known pediatric hospital in England, called Alder Hey, is working on integrating the HoloLens in the operating room. While performing surgery, the clinical team wear the headset that is able to overlay digital images onto their field of view, blending virtual reality with the real world ("Hololens in the operating room," 2018, para. 1). Using this technology, physicians and nurses can maintain a sterile field while accessing significant details from the patient information, view details of the procedure, including a CT or MRI scan. A physician shares in an interview, “Imaging a patient’s heart from the inside and from the outside is absolutely essential,” said Rafael Guerrero, a cardiac surgeon at Alder Hey. “I have to visualize that 3D view in my head in order to do this operation. You can display those images on a screen in the operating theater sometimes, but it’s not easily accessible; I can’t leave in the middle of an operation to go get more information about my patient. In many cases, the heart has already stopped in order for us to operate. Microsoft HoloLens and mixed reality will, in the future, enable me to have a patient’s scans in front of me while I’m doing the operation. If I can use technology to obtain that information, to see those images in front of me, that helps me tremendously and improves the outcome for my patient” ("Hololens in the operating room," 2018, para. 3).

As one of the latest technologies recently developed, there are limited data and studies on how HoloLens impact clinical practice. Questions have arisen regarding the accuracy of the images projected in relation to the real environment. Auvinet and colleagues studied the precision of the HoloLens headset, head, and hand motion measurement; although the error deviations from the calibration signal in various axis (lateral, vertical, anterior-posterior) range from 0.000 m +/- 0.005 m, the validation set deviations from the study range are 0.000 m +/- 0.01 m (Auvinet, Galna, Aframian, & Cobb, 2017, p. 176). Improvements are needed in the operating room, but the slightest deviation may pose serious results if a device is not checked or calibrated before use.
Device wearability and battery life may also pose limitations in the use of HoloLens. Students and clinicians who normally wear prescription glasses are not able to use it concomitantly with Hololens headgear due to fit issue; furthermore, use of HoloLens during long procedures and study sessions may be interrupted due to its short battery life range of three to four hours, if unplugged.

The benefits of using HoloLens outweighs the limitations granted that users can calibrate the device before a life-altering procedure. The main features of HoloLens are mixed reality (MR) based, which is different from virtual reality. In MR, clinicians’ tasks become more productive and efficient when surgeons can lay out the plan next to the operating table and stream imaging results next to the surgical site – a true start to end technology that can be utilized in clinical practice.
If practice goal is to place patients at the front-center of healthcare, future clinicians should also have robust preparation while in schools where anatomical and physiological learning is not limited to 2D and cadaver use. Organs, cells, and other structures under study should be viewed beyond 2D, and physiological functions made available via video streaming. Higher institutions who have adopted the use of HoloLens share increased productivity and enhanced learning from its educators and students alike.

An evaluation team has been finalized to collaborate on providing further information for this current innovation. Each team member was selected based on their primary strength, and in conjunction with the project lead’s top five strengths, using Strengths Finder 2.0.

Project lead’s top five strengths: Includer, Futuristic, Self-Assurance, Context, and Strategic

Team members: Alice, Chelsea, Daisy, Emily, and Matthew

· Alice was selected because she is an “Achiever.” Team members who possess this strength work hard to achieve the project’s goal and remain committed through the entire duration of a project. Her focus and hard work will motivate the rest of the team.

· Chelsea was selected to join the team because she possesses the “Command” trait. She will help the lead in facilitating the varying strengths and personalities among the team. Her presence and demeanor alone will assist in taking control of a situation during difficult times.

· Daisy was selected because of her “Developer” attitude to help the lead cultivate the potential in each team member. Because each team member has value to offer, it is critical to recognize these talents and celebrate even the small wins that could lead to bigger results.

· Emily was selected because of her “Empathy” towards others. Because a project is usually rigorous, it is important to ensure participation from others and minimize the possibility of losing members from the team. Emily is quick to sense other’s feelings and may be able to relate both positive and negative feelings or reactions to the lead, thereby improving group dynamics.

· Matthew was selected because of his ability to “Maximize” opportunities in any given situation by capitalizing on resources and talents on hand. This strength is relevant in teams working on a challenging project to elevate performance and produce excellent results.

(Gallup StrengthsFinder, 2018)
