Cause: They mixed up the units for each other, using kilograms instead of pounds, fuel gauge was not working
Effect: Plane carried half as much of fuel as they thought, they had to land the plane
Opinion: If countries were on the same unit system, there wouldn't be any confusion with measurements. Cause:
The workmen were measuring differently. One was measuring in Swedish feet, and the other was measuring in Amsterdam feet. The difference was 1 inch.
Effect:
The ship sunk and several people died.

Problem: The problem is that the telescope images were fuzzy.  

Cause: Main Mirror was sticking out 2.2 microns. 

Effect: It made the telescope's main mirror too flat. 

Opinion: It could have been prevented, if they had double checked it.
