In regards to your project, I really love the reflection component. This is such an important step to include in order for the students to really process these skills we're helping them develop. I also love the name "Success Tracker"! I also love the success tracker for the exact same reasons Katharine does. Your plan is well planned, and you did not leave much room for discussion :-). great job on creating both a teacher tracker and a student one. I find from personal experience that the teacher tracker works really well, and also keeps the teachers "honest" as well when it comes to monitoring a student's behavior
I did want to ask you something though, I believe I have the identical twin of your student in my class, and wanted your opinion.
