3. If you require a DBS check from scratch you'll need to come by the office in person to complete the form (we're located in Milton Court Level 2 at the end of the left hand corridor)

You will also need to bring Identification to support your application: 
I would recommend you bring Passport and your Driving licence (or birth certificate, if you don't drive) along with 2 letters/docs with proof of address on i.e. Mortgage statement, bank statement, credit card statement, P45/P60, Council Tax statement.
