DESCRIPTION OF THE INCIDENT

Incident Details:

While operating a lathe in Workshop No. 3, a metal workpiece slipped from the chuck and flew out of the lathe's position at very high speed. The machine was running at maximum speed, and although a safety guard was installed, the metal workpiece still caused a loud noise and slightly damaged the guard. Fortunately, no one was seriously injured, but the chuck and cutting tool were severely damaged.

CAUSES

The primary cause of the failure was improper machining. The workpiece was not properly clamped, resulting in the chuck being unable to firmly grip the workpiece during the initial entry of the metal workpiece. Additionally, excessively high feed rates and large depths of cut caused the workpiece to be ejected from the chuck.

RECOMMENDATIONS

To prevent similar incidents in the future, all operators need to be reminded to properly mount workpieces and tools. Increased safety training for apprentices and heightened concentration during machine operation are necessary. Strict enforcement of protective equipment and face shields is also required.
