I've spoken with Brandy and this is what she would like. It looks like some of you have been doing this already, so for those who didn't know about it, please begin to have your forms signed every time. Patrons may sign either the print out from SportsMan or the pre-printed "Center Pass Application & Liability Waiver" form. Please ensure that patrons are: Verifying that the information on the form is correct (name, address, phone, DOB, etc), Signing the form, and Confirming that the correct membership was selected and paid for. 18 years or older. Minors CANNOT sign their own paperwork. Also, if anyone ages 14-16 is signing up for a fitness room membership, please confirm that s/he has completed the youth certification program through Gina or Mele. Questions? Let me know. Thank you!
