         <description><![CDATA[<div>$3000 Base fee, $1,000 rescheduling fee, $500 for each level of care, and $300 for summary review and $3000 for full review. CARF will review the submitted application and may contact the organization for additional information is needed. CARF will schedule the survey to occur during the six months following <a href="http://carf.org/LOCcertification/conditions">completion of the application process</a>. CARF will determine the certification decision based on the survey and issue the decision. The certification decision is issued and made available to the organization, along with the Plan of Action form containing the cited elements, approximately 6-8 weeks after the survey.<br><br></div>]]></description>
