         <description><![CDATA[<div>Changed an elderly patient for a pelvis x-ray, he came in  wearing the gown wrongly i.e the split side was in front.<br>So once i asked him to lay down, everything could be seen (pirvate areas ) <br>I was so uncomfortable cos i was the only one at the room, the radiographer supervising me was behind the screen. Had to call for help. <br>I realised he was wearinga hearing aid, so i learnt effective communication in difficult situations. So, next time i will assess the patient properly and explain what ever i want them to do after judging their situation. </div>]]></description>

         <description><![CDATA[<div>What?  A radiographer give me a dressing down because the wrong exposure was selected and I nearly exposed.  Fortunately, I paused and checked which highlighted the error.<br>So what?  This could have been a potential radiation incident. The radiographer was actually the one who selected the wrong exposure.  I was blamed for this and didn't correct the radiographer.<br>What now?  I always double check the exposure setting even if a qualified radiographer tells me they have done it.  In future I will professionally address the situation.</div>]]></description>

         <description><![CDATA[<div>A patient with dementia came needing a chest x-ray. They came in with a carer, and wanted to go everywhere where their carer went. Found it really hard to get the patient to stay still when everyone went behind the screen.<br>In the end had to ask the carer to give the patient loads of reassurance, and had to move really quick in order to get an image. <br>Reflecting on this situation would help me explore ways of  getting around such difficult cases and reduce possibilities of re-exposure.</div>]]></description>
