I think the teacher should be more aware when there are students who might be subject to bullying. Perhaps a new student needs more support. The teacher might have been more aware had he been circulating through the room. This is where an educational assistant can be really valuable as an extra set of eyes, looking out for a student's well being.

I feel as if the students need to be empowered to speak up and let people know if they are bullied, but I understand that they are reluctant. Sometimes a student will be fearful that "telling an adult," is like tattling and will only result in them feeling more like an outsider. I do think the teacher (Librarian?) in this case could have been a little more forceful in asking about the incident. It seemed that the girl probably wanted to share the truth but was scared. I know that students have to feel safe to be able to speak their minds and that's very difficult to accomplish in all cases. In scenarios where the bully might be anonymous "e.g. cyber bullying" it might be even more difficult to intervene.

If I was at the scene as the Librarian was I would address the issue on the spot. Regardless of the bullies having left the room I'd make sure to bring them all together. Something that often works with students is telling them how life works, not everyone you work with or study with is going to like you. Another thing, life goes on whether you have two friends or a bunch of friends.

The teacher's role here should have been to be more persistent and make sure to get involved. As a mandated reporter it'd be safer to have the incident reported in order to avoid future confrontation between the students.
