         <description><![CDATA[<p>In the community you will come across a wide range of learning opportunities, including wound care which may be simple, or can be complex, such as compression bandaging or post-op wound care, such as suture removal or wound packing. We carry out catheter care and maintenance solutions, including changing male/female and suprapubic catheters. We also carry out continence assessments. We carry out venepuncture and give injections such as b12, flu, prostap or GCSF injections, as well as end of life stat doses. We carry out diabetic care which includes taking blood glucose readings and giving insulin. Treating hypo/hyperglycaemia and escalation if needed. We carry out observations routinely or in emergency situations. We do regular palliative care visits and set up syringe drivers. We manage HICK/PICC lines, and gastrostomy tubes.</p><p>We liaise with multi-disciplinary teams such as Gp, out of hours services, hospice nurses, dietitian’s, social workers, tissue viability and lymphoedema and regularly send referrals to these.</p>]]></description>

         <title>Commonly used definitions in wound care</title>

         <description><![CDATA[<p>A wound is described as a break in the skin and underlying tissue. </p><p>Bio-film- a colony of microbes in the tissue.</p><p><br/></p><p>Colonisation- a mass of bacteria formed by cells, that multiplies in the surface of the wound in warm conditions.</p><p><br/></p><p>Dehiscence- total or partial separation of the wound edges whilst in the early stages of wound healing.</p><p><br/></p><p>Eschar- dead tissue that sheds or falls away from the skin.</p><p><br/></p><p>Excoriation- tissue damage that is often bright red in colour.</p><p><br/></p><p>Exudate- the amount of fluid coming from the wound.</p><p><br/></p><p>Granulation- new tissue being formed on the wound surface.</p><p><br/></p><p>Maceration- cause by excessive amounts of moisture, leaving the skin wet and white in colour.</p><p><br/></p><p>Necrotic- dead skin/tissue, black in colour. Will often need to be debrided.</p><p><br/></p><p>Slough- a yellow thick covering over the wound.</p><p><br/></p><p>Superficial- a cut that does not penetrate the skin.</p><p><br/></p><p>Oedema- swelling that looks shiny and full of fluid.</p><p><br/></p><p>Venous leg ulcer- caused by circulatory problems. Slow to heal and causes blood flow to become blocked which then pools fluid in the legs.</p><p><br/></p>]]></description>

         <title>Pressure area grading</title>

         <description><![CDATA[<p>A pressure area is an injury to the skin and underlying tissue.</p><p><br/></p><p>Moisture lesions- caused by moisture and can cause partial skin loss.</p><p><br/></p><p>Suspected deep tissue injury (SDTI)- May be purple in colour or have a blood filled blister with depth unknown.</p><p><br/></p><p>Grade 1- intact skin with non blanching redness.</p><p><br/></p><p>Grade 2- partial skin loss of the epidermis and dermis layer.</p><p><br/></p><p>Grade 3- full thickness tissue loss where subcutaneous fat may be visible.</p><p><br/></p><p>Grade 4- full thickness tissue loss where bone or tendon may be visible, and may be covered by slough or eschar.</p><p><br/></p><p>Ungradeable- full thickness loss which is obscured by slough and/or eschar.</p>]]></description>
