Why is there a lack of loyalty among millenials? Is this an internal organisational issue or an external cultural issue?  Does it point to poor recruitment policies – at which stage – advertising, selection, recruitment, induction, training, promotion, etc.

1.)             Do you intend to stay with Maples for the foreseeable future?

I wouldn’t ask this question at the start of your interview, but it might come out/be asked later in the interview – they might become suspicious, and unlikely to be honest on that one.

Your second q. Is a bit of a leading q. Perhaps ask if they think there is a high turnover in company/department first, then were they aware of this before starting (but only if they are a recent recruit).

Is the first part of question 5 leading the respondent to answer in a certain way?  I think you need to break the rest of this into sub-questions – not asked all at same time as this may be confusing for interviewee.  Look at these questions again and see if you are asking more than one question in one.

How are you going to develop pie or bar charts from the answers, and how does this conform with quantitative research?  You won’t have enough quantitative data from a representative sample to have any type of meaningful charts.  Review this in terms of what quantitative method can be used to develop your overall research objectives.
