         <description><![CDATA[<div><strong>What would you put in place?</strong><br>Knowing where people go to find the information e.g shops, facebook groups etc<br>Having a "<mark>local expert</mark>" who knows the area, how to refer to services and where to go. Not just a website but an actual person. The area covered by the LA and trust is large and made of a few smaller villages and many people use social networks as a main source of communication. Maybe a local app that people can ask either questions on (like NextDoor app- private social network for neighborhood community).<br><br><strong>How would you exercise your influence?</strong><br>Meeting with groups that are already running and bringing them together in one place.<br>Meeting and <mark>connecting community leaders</mark><br><br><strong>What could you do knowing the constraints? <br></strong>Recognise the concerns and hold a public forum to discuss issues. Volunteer committee with specific actions/goals and use social media to feed into this- in places where the community is spread out or across a large area. <br>Making the first step can scary- befriending scheme/ buddy systems/ care coordinator. <mark>Bringing people together with a purpose rather than just for the reason of "connecting"</mark>- a task/ goal etc <br>Manage the risk that is highlighted by the team and convincing the team that this is an investment in recovery. <br><strong>What do you understand by the local community?<br></strong><mark>Not just the people who live near each other but also people who feel connected together </mark>such as sport clubs, people who went to school together, politics/beliefs , local pubs/cafe, culture, religious, dog walkers etc.<br>Being part of a community has it pros and cons and can be drain on your resources as well as a benefit. <br>Informal support as you need it.<br>Range of ages.<br><mark>Where people live can become part of their identity.  </mark><strong> </strong></div>]]></description>

Make it multidisciplinary/ more joint up work other professionals can attend and provide their input<br>do beta tests/pilot studies before full implementing- make service users the experts in their experience and report back. <br>Funding- from businesses e.g coffee morning (MacMillan). <br>Travel- car pooling, use third sector organisations.- utilise different resources <br><br>What do you understand by the local community?<br>Community- a sense of belonging and connection.          <description><![CDATA[<div><strong>Sea </strong>- Cleanup for the beach, taking pride in your community. Really easy to implement as people already doing this, could implement a raffling reward system for stimulus.<br><br><strong>Green Spaces </strong>- Make better use of the many Parkruns in the area and try to encourage people who are mobile to get out, have something to aim for (PBs). Walking has many health benefits. <br><strong><br>Photography</strong> - Utilising the resources of the photography school locally. Tied into identity and could be inexpensive. Could give people who are homeless cameras and put on a display!<br>- Specifically using phones so that everyone can get involved<br>- Potentially contact the students at the photography school so that they can put on workshops<br>- Social events where people can come to appreciate each others' pictures and ideas<br>- Shows could be monthly or fortnightly with access to everyone so that anyone can attend<br>- Themes every fortnight or week so that if you miss one then you don't mind or get left behind<br>- One week 'on' where you go out and take pictures (get people started thinking about that week's theme) and then next week show.<br>- Photobook to collate all the winning pictures in hard form and to act as a reminder of good times, or potentially a calendar.<br>- Online social media <br>- Service user expert to sit on judging panel or help run workshops or classes<br>- Link in to existing organisations for advice, support, etc. <br><br>Need:<br><br>Space/Projector/FB or other social media groups/Moderator (specialist)/ Library?<br><br>Perhaps approaching existent groups that have some sort of standing, attempting to participate with them and help expand their remit rather that starting something from scratch. </div>]]></description>

It's a great resource for people and professionals to find community groups and resources related to their interests.          <description><![CDATA[<div><br></div><h1>Connecting People Database Project Plan: </h1><div> </div><div>Spec: A searchable database of community resources, relevant to service users. (Not rocket-science but really needed- this used to be produced by a charity, but no longer, and meanwhile all STR workers in our team have is a disorganised filing cabinet! </div><div> </div><div>Can make it available to social prescribers when they come into Redbridge. </div><div> </div><div>Rationale: </div><div>-       If everyone is able to do simple signposting then the STR clinic can be more specialised and  STRs freed up to do graded exposure etc  </div><div> </div><div> </div><div>This does not exist </div><div> </div><div>1)    Increase our understanding/confidence/fidelity Connecting People </div><div>2)    Discuss who is doing what </div><div>3)    Meet with STR workers- after CCM- explain connecting people to establish a legacy </div><div>4)    Identify key list of “informers” e.g. CVS- Jenny Ellis </div><div>5)    Build excel spreadsheet </div><div>6)    Explain CP/spreadsheet in monthly business meeting </div><div>7)    Follow up (in zoning?)/plan to ensure the team uses it </div>]]></description>

         <description><![CDATA[<div>Approach existing groups that have been established and are untapped resources used by the CMHT e.g. the Bedford 20s &amp; 30s Women’s Group (366 members) or Bedford and Stewartby Laughter Club/ Bedfordshire Film Makers and discuss potential signposting for individuals to their group and/or expansion. </div><div> </div><div>Have a form at the reception in the CMHT and other services e.g. GP, drug and alcohol, recovery college etc. (and liaise with reception and CC’s to prompt this) and pens asking individuals/family members what current groups/community/activities/hobbies they are involved in AND if you do not go, why/what stops you? What could be done to improve this/support you to join group/community/activity etc./what factors would help you go <em>if </em>you wanted to? please list the groups you know of or may be interested in etc.</div><div>-       Have the form anonymised and indicate where filled out </div><div>-       Also, encourage members of the team to ask the people they are working with about this and when gathered a list of resources/information around this, can send email to all members of team/other teams including information about signposting (and will send the information to GP, drug and alcohol etc. with the information from the people using their services </div><div> </div><div>Establish a signposting shop whereby individuals/family members could approach/be approached to discuss community resources. If people gave information and we were unaware of the resources, can leave email to be contacted or return to the shop after we have had time to do some digging. </div>]]></description>

         <description><![CDATA[<div><strong>Community champions within Crisis teams. <br> - </strong>Current issue with signposting to other services without follow up and lack of knowledge around community resources. <br>- Plan to make connecting to community groups and resources a fundamental part of Crisis intervention rather than just signpost for after discharge. <br>- Inviting community groups and third sector services to business meetings to build knowledge. <br>Invite mentors/befrienders to visits with service users to link them up. <br>- Champion community resources as part of intervention along with the medical model. </div>]]></description>

         <description><![CDATA[<div>- Already available: online portal of groups and activities in area (set up by the council), though we are unsure how many people in the team are aware of this.<br>- Community Connectors - paid staff from local wellbeing hub connecting people to opportunities in community. We will meet with CC to try to find out about which activities people generally ask for and whether this need is currently met to determine if there is a particular activity we could run.<br>- Find out how the portal is being run to see if opportunity for a community run section - 'reciprocity'!<br>- More generally, promote the online portal and the need to 'think community' with people we work with rather than immediately referring them/signposting them elsewhere. </div>]]></description>

         <description><![CDATA[<div>- Community Assessment and Recovery Service (CARS)<br><br><strong>(1) Co-production / Co –development with community service  (Building relationships). </strong></div><div><br></div><div>As students we should contact NEW OPPORTUNITIES WORCESTERSHIRE (NOW) Team to arrange an meeting to establish;  </div><div><br></div><div>(a)How MHSWs and other professionals can promote their services to individuals using CARs teams. <br><br></div><div>(b)The ways can work together to co-produce bespoke activities for CARs teams.  <br>  </div><div>(c)The key is to build a good relationship with this service.<br><br>NOW have flexibility to design activities tailored to particular SU<br> <br> (<strong>2) Online Directory – </strong>create a list of community services and activities (updated monthly) – share with other professionals and encourage them to refer/work with the identified community organisations.      <br>                                                                                                                                                                                                                                                                            <strong>(3) Ultising Re-enablement Worker – ‘</strong>touch base’ with the re-enablement worker. Find out the extent of how much, the worker is supporting individuals to engage in the community (so our work does not overlap). </div>]]></description>
