-      Non-electronic resources and information sharing e.g. community boards.
- Someone with dedicated role of increasing access to community resources.

What is it that you could practically do to establish community inclusion?

-      Organising transport for people to local parks/green spaces. 

-      Online forums/groups on social network platforms for community activities or sports. 

-      Community meals 

-      Locality working 

-      Going to people – accompanying people when they are socially anxious/doing something for the first time.

What would you do knowing these constraints?

-      Public spaces – finding out when they are available to use and sharing this info with service users e.g. making connections between stakeholders/community leaders in the borough and service users by hosting meetings.

How would you exercise your influence in establishing community inclusion which would favour people in that community who need community support and inclusion? 

- Influence people to volunteer and delegate work

What could you do knowing the constraints

- Constraints: time, money, knowledge, lack of engagement, risks, competence, expertise 

- delegate, share the load

- building relationships with institutions, small businesses, people with history/experience 

- publicity/advertisement

What do you understand by the local community  

- something that binds you together – space, location, race, culture, interests, shared values/attitudes

- something with a focus? Something that provides resources – an exchange of resources. 

- Utilise service user opinions – what resources have they been using that professionals may not know about?

- We could get this information from service users on our caseloads

- We can get information when doing initial assessments – by asking questions ‘what services do you engage with in your communities’

- These resources can then be fed back into the team through MDT meetings 
- This then means that resources for specific communities (e.g. cultural minorities) can be made aware to the team 
- It helps us to be aware of the blind spots that we as professionals may have in the communities

-      We present new services that we find at team meetings. Also ask workers from community and voluntary services to send in information for us to present on their behalf. 

-      Ask service users for their feedback on services they attend and present this to our team at team meetings. 

-      Consider creating a spreadsheet in the team of local voluntary and community  services.

-      Raise issue of information sharing on services at the next Student Forum meeting. Suggest creating an online shared document e.g. on One Note to share information between students in the boroughs. 

o  If we feel this is successful/useful consider presenting information on services to management to be added to an online directory for everyone to access. 

-      Sharing link to directory of services with professionals and service users. 

 

Spec: A searchable database of community resources, relevant to service users. (Not rocket-science but really needed- this used to be produced by a charity, but no longer, and meanwhile all STR workers in our team have is a disorganised filing cabinet! 

 

Can make it available to social prescribers when they come into Redbridge. 

 

Rationale: 

-       If everyone is able to do simple signposting then the STR clinic can be more specialised and  STRs freed up to do graded exposure etc  

 

 

This does not exist 

 

1)    Increase our understanding/confidence/fidelity Connecting People

3)    Meet with STR workers- after CCM- explain connecting people to establish a legacy 

4)    Identify key list of “informers” e.g. CVS- Jenny Ellis

6)    Explain CP/spreadsheet in monthly business meeting 

7)    Follow up (in zoning?)/plan to ensure the team uses it 

 

Have a form at the reception in the CMHT and other services e.g. GP, drug and alcohol, recovery college etc. (and liaise with reception and CC’s to prompt this) and pens asking individuals/family members what current groups/community/activities/hobbies they are involved in AND if you do not go, why/what stops you? What could be done to improve this/support you to join group/community/activity etc./what factors would help you go if you wanted to? please list the groups you know of or may be interested in etc.

-       Have the form anonymised and indicate where filled out 

-       Also, encourage members of the team to ask the people they are working with about this and when gathered a list of resources/information around this, can send email to all members of team/other teams including information about signposting (and will send the information to GP, drug and alcohol etc. with the information from the people using their services 

 

Establish a signposting shop whereby individuals/family members could approach/be approached to discuss community resources. If people gave information and we were unaware of the resources, can leave email to be contacted or return to the shop after we have had time to do some digging. 

As students we should contact NEW OPPORTUNITIES WORCESTERSHIRE (NOW) Team to arrange an meeting to establish;  

(a)How MHSWs and other professionals can promote their services to individuals using CARs teams. 

(b)The ways can work together to co-produce bespoke activities for CARs teams.  
  

(c)The key is to build a good relationship with this service.

NOW have flexibility to design activities tailored to particular SU
 
 (2) Online Directory – create a list of community services and activities (updated monthly) – share with other professionals and encourage them to refer/work with the identified community organisations.     
                                                                                                                                                                                                                                                                            (3) Ultising Re-enablement Worker – ‘touch base’ with the re-enablement worker. Find out the extent of how much, the worker is supporting individuals to engage in the community (so our work does not overlap).
