Inspire Upper Airway Stimulation Pacemaker Device

https://www.youtube.com/watch?viwacxD_v91w

Through the advancement of informatics and biomedical engineering the Inspire Upper Airway Stimulator has come into being. This implantable device is about the size of a pacemaker. It uses electronic stimulation to cause the base of the tongue to lift in obstructive sleep apnea. This opens the airway and does not awaken the patient. The implanted device and lack of any type of facemask or external apparatus leads to compliance of the sleep apnea patient. Since most sleep apnea patients are middle aged, the eventual surgical replacement of the generator would be a negative. But the positives clearly outweigh any negatives.

Sleep apnea affects adults, usually obese males, and children with big tonsils. Symptoms are snoring and being overly sleepy in the daytime hours. Breathing may be shallow or stop while sleeping. This can occur a few or many times during a night.

Obstructive sleep apnea is the most common form of sleep apnea. The airway collapses or gets blocked and causes a temporary obstruction which startles the patient from deep sleep .
Central sleep apnea is not as common. This form arises from the brain. The signal is not sent to the diaphragmatic and accessory muscles to do the work of breathing. The patient makes no effort to breathe for periods of time. Patients with central sleep apnea do not usually snore.

Sleep Apnea: The incidence of sleep apnea among United States veterans has increased in the 50-64 year age group from 3.7% to 8.1%. The incidence has increased 6-fold. Speculative cause is an increase in psychological distress, unmet mental health needs, and asthma. Vets with mild or moderate increase in psychological distress over the last year had a 61% higher chance of having sleep apnea. Those with serious psychological distress over the last year had a 138% increased odds of sleep apnea. If he has asthma, the odds jump to 256% higher. Asthma onset may be in the field from environmental risk factors or not diagnosed prior to enlistment deployment.

Some have eventually given up on the CPAP due to the inconvenience and lack of tolerance of the device. I stronglyfeel this procedure will lead to increased compliance and decrease the many co-morbities of sleep apnea.

In the innovation of new products, NIs are needed to take the stated problem (sleep apnea) and coordinate with nursing staff, IT, medical staff, and others to define the problem, develop clinical tools for gathering data regarding the problem, and refine the information for presentation of the problem to stakeholders. In the instance of sleep apnea, for example, the ICU nurse could report noting a high number of patients with sleep apnea. This could trigger the involvement of NIs to assist in the design of a computer audit tool to pull data and make it the basis of reporting the noted trend. The data gathered could spark an idea in an entrepreneurial person to devise a solution, such as the Inspire Upper Airway Stimulation Device. NIs are able to move between nursing science, computer science, and informational science. They are able to provide the needed support to education and research using nursing and computer knowledge.
