Sick; aka rad, bad, cool, killer, gnarly, epic refers to something that is amazing or impressive.  This term is often used by unruly skater or snowboarder type teenagers ;)

What teenagers mean when they sick is that it's amazing and really cool or rad as heck. If someone does a cool trick on a skateboard or bike that'd qualify as sick. If someone plays a really cool solo on a guitar i'd say that's sick.
