I think it was both. The carving was placed there specifically so it wouldn't fall down to the area of the brig, and, although there were small winds, the candle hadn't flickered out from the small blows. Charlotte may not recognize who put the candle out (Chapter 6 end), but only a small glimpse wouldn't be enough to make any distinct comparison to a crew member. Maybe Mr. Keetch was jealous of Charlotte for how she was being treated (Glanced back) and ordered one of the other crew members to do that. (They were probably loathing her for making them seem inferior when Captain Jaggery escorted her out.) Or, it might have been the one man that she saw when she was being brought on the ship by Mr. Grummage. The man who climbed on the ship and Charlotte noticed his form up in the sails and net. We've seen nothing about him, nor found any hints as to whom he is. Maybe that man causes other problems around the ship. That might be what the last sentence of chapter 7 means. Charlotte was hoping to not have to use the dirk and keep it to please Zachariah and Captain Jaggery. That seems to hint her plans shall not move accordingly. Also, Jaggery might have put the sails down to catch a wind that was not yet there. He does have a reputation as a Captain that has quick voyages.
