<title>Crusted scabies was first described among leprosy patients in Norway in 1848 and thus is historically known as Norwegian scabies. It is a severe, debilitating disease characterized by large numbers of mites, high immunoglobulin E (IgE) levels, peripheral eosinophilia, and the development of hyperkeratotic skin crusts that may be either loose, scaly, and flaky or thick and adherent.</title>

<title>Who gets crusted scabies? People whose immune system is significantly compromised can develop crusted scabies. Patients who are already suffering from scabies have also been known to develop this more severe version. Those who are most at risk include: the elderly, young children, people with disabilities, patients who have a condition that affects their immune system, e.g. HIV or AIDS, undergoing chemotherapy, transplant recipients.</title>

<title>This condition is highly contagious through direct contact. To minimize the spread of crusted scabies, those infected should not share clothes, bedding or other personal items with other people. Areas where the patient has been should also be carefully cleaned to eliminate traces of the disease. </title>

<title>Diagnosis requires a thorough history of the present illness and physical exam. Confirmation of the diagnosis requires the isolation of the mite, eggs, or mite feces. An alternative diagnostic method that can be applied is the burrow ink test, in which suspicious papules are marked with ink and then wiped off with an alcohol pad to remove ink left on the surface. A positive result occurs when the ink tracks down the mite burrow, forming a characteristic zigzagged line visible to the naked eye.</title>

<title>Crusted scabies typically presents with hyperkeratotic plaques that may be fissured and can have associated erythema. Complications may include other, more life-threatening situations. The tendency of the skin to flake off easily increases the risk of bacterial disease such as staphylococcal, streptococcal, cellulitis or impetigo. </title>

<title>Treatment consists of both prescription oral and topical medications. First line therapy for crusted scabies includes topical permethrin and oral ivermectin. Duration of treatment depends on the severity of the disease. </title>

<title>Patients may also be treated with keratolytic agents to help remove the affected skin. As crusted scabies can be difficult to heal, the process may need to be repeated to shake the condition off.</title>
