         <description><![CDATA[<p>The <strong>Responsible Officer</strong> is a senior clinician in a designated body who ensures that the doctors for whom they act in this nominated capacity continue to practice safely and are properly supported and managed in maintaining their professional standards.</p><p>When a doctor is involved in an SI their Responsible Officer needs to be notified.&nbsp;</p><p>This is usually the <strong>Postgraduate Dean</strong>. </p>]]></description>

         <description><![CDATA[<p>Form 'R' constitutes evidence for the GMC that you have considered involvement in significant incidents. </p><p>Deciding whether an incident is significant is open to interpretation and judgement and you should discuss this with your ES. Broadly you need to record any complaints and significant events that were <strong>formally </strong>investigated by your employing organisation / Health Education England / professional body, or that are currently unresolved since your last ARCP. </p><p><br/></p><p>This includes :</p><p><strong>MNSI </strong>investigations</p><p><strong>PSIIs -</strong> may include 'never events'</p><p>any <strong>complaints</strong> that you have been named in</p><p>incidents involving the <strong>Coroner</strong> - usually unexpected deaths</p><p><br/></p><p>You do not need to declare involvement in IIR or AAR processes or participation in an MNSI investigation where you were not directly involved in providing care.<strong><br></strong></p><p>This will be part of your sign-off discussion prior to the ARCP, and the issues may be discussed at your ARCP.&nbsp;</p><p>The ARCP panel is interested in what you have learnt as well, and you must reflect on any significant events, complaints, and investigations in your e-portfolio.</p>]]></description>
